Rupture of Aortic Dissection During Attempted Transesophageal Echocardiography

Abstract: Transesophageal echocardiography (TEE) provides a rapid and accurate diagnosis in patients with dissection of the thoracic aorta. The procedure is considered to be safe. We report a case where rupture of a dissecting thoracic aneurysm occurred during attempted TEE in a patient, who had been clinically stable for 1 week. TEE was performed because aortic valve endocarditis was suspected, and the procedure did not include close surveillance of the blood pressure. Careful manipulation of the probe, adequate sedati… Show more

“…19 There are anecdotal reports of deterioration of patients with aortic dissection, during TEE, possibly related to discomfort, gagging or distress. 20 However, no mortality has been reported in larger studies. 16,17,19 Moreover, given the mortality rate of 1-2% per hour, there is a high risk of fatal deterioration while transferring patients for, or during, CT or MRI.…”

“…193 Considering that 90% of the patients survive if they reach invasive treatment (open surgery and SG deployment), it is desirable to avoid misdiagnosis and resulting delays in transportation by performing a quick and simple echographic examination. However, although the diagnostic ability of TEE is comparable to that of CT, 194 it may induce rupture due to the stimulation of insertion, 195,196 and it is not considered optimal for initial diagnosis.…”

“…2 However, insertion and manipulation of a TEE probe may lead to a variety of complications. [3][4][5][6][7][8][9][10] Trauma associated with insertion and maneuvering of a TEE probe may predispose a patient to develop airway swelling while in the sitting position.…”
