Ruptured corpus luteum cyst in early pregnancy: A case report

Abstract: Since the diagnosis of early pregnancy and its complications can be misleading, in unclear cases, we support the expectative "wait and see" management consisting of serial hCG testing and repeated ultrasound examinations. Avoidance of uterine curettage in such unclear cases would further reduce the possibility of normal early pregnancy interruption.

“…In the first case, the pregnancy was immediately terminated after completion of the laparoscopic procedure [5]. In the more recent case, after successful laparoscopic AHP treatment the pregnancy continued to term and resulted in the birth of a healthy newborn [6]. Additionally, we found one case of laparoscopically evacuated hemoperitoneum (1 L) in an early pregnancy (8 th GW) of unknown localization, with spontaneous hCG decline within 2 weeks following the surgery [16].…”

“…In most reported cases of AHP in pregnancy, surgery was performed via laparotomy [2][3][4]. We identified only 2 cases of laparoscopic AHP treatment due to CL cyst rupture in early viable intrauterine pregnancy [5,6]. In the first case, the pregnancy was immediately terminated after completion of the laparoscopic procedure [5].…”

“…In unclear cases, some reports recommend a “wait and see” approach constituting serial b-HCG levels and repeat ultrasound imaging [7]. In our case the patient had a positive pregnancy test with a litre of blood in her abdomen.…”

“…However there are reports to suggest the coincidental presence of a corpus luteal cyst with an intrauterine pregnancy [7]. The latter is a clinical conundrum that can be most challenging for the clinician in determining treatment.…”
