Patients with Post-COVID syndrome (PCS) are frequently referred for cardiologic evaluation. We assessed cardiac function and biomarkers in relation to functional status and fatigue in patients with PCS. This prospective single-center cohort study included 227 patients with persisting symptoms after COVID-19 infection. Most frequent complaints were fatigue (70%), dyspnea (56%), neurocognitive symptoms (34%) and chest pain (28%). Standardized questionnaires were used to assess Post-COVID-Functional-Scale (PCFS) and fatigue (MFI-20). The fatigue severity was inversely related to age and did not correlate with cardiovascular diseases, echocardiographic findings, or biomarkers. Similarly, mild to moderate functional impairment (PCFS 1–3) did not correlate with cardiovascular alterations. However, the subgroup of patients with significant functional impairment (PCFS = 4) had more frequent cardiovascular comorbidities, biomarkers and impaired global longitudinal strain (GLS). Patients with elevated troponin T showed abnormal GLS, reduced left ventricular ejection fraction and impaired tricuspid annular plane systolic excursion. The majority of patients with PCS shows a normal cardiac function. Only the small subgroup of patients with severe functional impairment and patients with elevated troponin T is at risk for impaired cardiac function and likely to benefit from specialized care by a cardiologist.

Objective This S2k guideline of the German Society for Gynecology and Obstetrics (DGGG) and the German Society of Perinatal Medicine (DGPM) contains consensus-based recommendations for the care and treatment of pregnant women, parturient women, women who have recently given birth, and breastfeeding women with SARS-CoV-2 infection and their newborn infants. The aim of the guideline is to provide recommendations for action in the time of the COVID-19 pandemic for professionals caring for the above-listed groups of people. Methods The PICO format was used to develop specific questions. A systematic targeted search of the literature was carried out using PubMed, and previously formulated statements and recommendations issued by the DGGG and the DGPM were used to summarize the evidence. This guideline also drew on research data from the CRONOS registry. As the data basis was insufficient for a purely evidence-based guideline, the guideline was compiled using an S2k-level consensus-based process. After summarizing and presenting the available data, the guideline authors drafted recommendations in response to the formulated PICO questions, which were then discussed and voted on. Recommendations Recommendations on hygiene measures, prevention measures and care during pregnancy, delivery, the puerperium and while breastfeeding were prepared. They also included aspects relating to the monitoring of mother and child during and after infection with COVID-19, indications for thrombosis prophylaxis, caring for women with COVID-19 while they are giving birth, the presence of birth companions, postnatal care, and testing and monitoring the neonate during rooming-in or on the pediatric ward.

Zusammenfassung Ziel Die Post-COVID-Sprechstunde (PCS) wird im Rahmen eines umfassenden Behandlungsangebotes der gesetzlichen Unfallversicherung für Post-COVID-Patient*innen durchgeführt, um individuelle Empfehlungen für die weitere Versorgung festzulegen. Ziel der Untersuchung war die Erfassung der Hauptsymptome sowie die damit verbundenen Einschränkungen der sozialen und beruflichen Teilhabe, um Konsequenzen für die ambulante Rehabilitation abzuleiten. Methodik Neben einer ärztlichen Untersuchung sowie einem psychologischen Konsil wurden zahlreiche Assessments zur Evaluation des biopsychosozialen Gesundheitszustands durchgeführt. 373 weibliche (82,2%) und 81 männliche Patient*innen im Alter zwischen 40 und 60 Jahren aus den Berufen der Gesundheits- und Pflegedienste, der Erziehung und Pädagogik nahmen seit April 2021 an der PCS teil. Ergebnisse Nahezu alle untersuchten Patient*innen (98,2%) gaben Fatigue als Kardinalsymptom ihrer Post-COVID-Beschwerden an, in über 73% der Fälle in Kombination mit subjektiv erlebten Einschränkungen der Hirnleistungsfähigkeit. Die Dauer der Symptomatik persistierte dabei sowohl bei den weiblichen als auch männlichen Versicherten durchschnittlich seit 14–15 Monaten. Somit sind über 85% der Gesamtstichprobe als Fälle chronischer Erschöpfung (Fatigue Skala) einzustufen. Die Schwere der Fatigue beeinflusst zudem proportional die Lebensqualität (SF-36), Gefühle von Angst und Depression (HADS), die psychische Widerstandskraft (RS-13) sowie motorische Parameter wie die maximale Greifkraft und die Ausdauerleistungsfähigkeit. 54,3% der Versicherten erhielten zudem eine psychische Verdachtsdiagnose und 38,1% die Empfehlung zu einer weiterführenden neuropsychologischen Diagnostik. Schlussfolgerung Zur weiteren Behandlung des Leitsymptoms der chronischen Fatigue ist eine multimodale und interdisziplinäre ambulante Rehabilitation zu empfehlen, die sich an der Behandlung der Diagnose des Chronischen Fatiguesyndroms (ME/CFS) und somit insbesondere an einem psychoedukativen und weniger an einem kurativen Therapieansatz orientieren und Nachsorgestrategien berücksichtigen sollte. Gesicherte psychische Störungen und neuropsychologische Defizite sind begleitend zu behandeln.
