Sacral neuromodulation and pregnancy: Results of a national survey carried out for the neuro‐urology committee of the French Association of Urology (AFU)

Abstract: In 27 pregnancies established during SNM for LUTS, 18.5% of patients deactivated their case before pregnancy and the others switched it off during the first trimester. Three-quarters of women with CRU had urinary infection. No adverse effects on fetuses were found. SNM effectiveness deteriorated in 20% cases after childbirth.

“…After delivery, SNM efficacy must be checked to exclude displacement or electrode damage. Roulette and colleagues reported that SNM efficacy deteriorated in 20% of women postpartum, irrespective of the mode of delivery [ 27 ]. In most of these cases, the reprogramming of the device was sufficient to restore its proper functioning, with explantation hardly ever being required.…”

“…For this reason, the parent company as well as numerous urological and gynecological associations-including the International Urogynaecological Association (IUGA)-suggests keeping implantable pulse generators (IPGs) off during pregnancy, as "the effect of neuromodulation on pregnancy is largely unknown." A national survey carried out by emailing all the urologists listed in the Interstim National Registry in France showed that deactivation of the device was advised in 18.5% of cases while trying to conceive and during the first trimester in the remaining 81.5% cases [4]. Two systematic reviews are currently available on the topic.…”

“…Dans ce dernier cas, chez une patiente implantée, il est recommandé d'arrêter la stimulation dès le début de la grossesse et probablement dès l'arrêt de toute contraception. Il a été récemment rapporté qu'après l'accouchement, la remise en route du système permettait de retrouver la même efficacité dans 80 % des cas [41].…”
