Safety and continued use of the levonorgestrel intrauterine system as compared with the copper intrauterine device among women living with HIV in South Africa: A randomized controlled trial

Abstract: BackgroundWomen living with HIV (WLHIV) have lower rates of contraceptive use than noninfected peers, yet concerns regarding contraceptive efficacy and interaction with antiretroviral therapy (ART) complicate counseling. Hormonal contraceptives may increase genital tract HIV viral load (gVL) and sexual transmission risk to male partners. We compared gVL, plasma VL (pVL), and intrauterine contraceptive (IUC) continuation between the levonorgestrel intrauterine system (LNG-IUS) and copper intrauterine device (C-… Show more

“…A study that investigated the role of two new low‐dose LNG‐IUS (8 and 13 µg/day) over 3‐year follow up reported that the risk of PID was the same in nulliparas and paras (crude incidence 0.1% vs 0.6%) and between women aged 18‐25 years and those aged 26‐35 years (crude incidence 0.2% vs 0.6%) 18 . Another study that investigated the PID incidence rates in women with HIV infection reported a rate of 1.2/100 W‐Ys (95% CI 0.3‐4.6) among LNG‐IUS users compared with a rate of 0.7/100 W‐Ys, (95% CI 0.1‐4.9) ( p = .68) among Cu‐IUD users 33 …”

“…With the two low-dose LNG-IUS (8 and 13 µg/d) nulliparous women showed the same risk as parous women (crude PID incidence: 0.1% vs 0.6%, respectively); 18 Women living with HIV users of the LNG-IUS had low incidence of PID compared with the Cu-IUD users; 33 Conception rates up to 1 y after LNG-IUS removal were similar between nulligravidas (86.8%) and parous (85.4%) women. 35…”

“…Recent experience has shown that the hormonal IUD could be an important addition to the contraceptive method mix in the region. A study among postpartum women in Kenya and a trial with women living with HIV in South Africa showed high satisfaction and continuation rates with the hormonal IUD that were comparable with or superior to other LARCs [ 8 , 9 ]. Qualitative interviews in Ghana, Kenya and Nigeria revealed favorable perceptions of the hormonal IUD among key opinion leaders, providers, and early adopters [ 10 14 ].…”
