Safety and effectiveness of mass drug administration to accelerate elimination of artemisinin-resistant falciparum malaria: A pilot trial in four villages of Eastern Myanmar

Abstract: Background: Artemisinin and partner drug-resistant falciparum malaria is expanding over the Greater Mekong Sub-region (GMS). Eliminating falciparum malaria in the GMS while drugs still retain enough efficacy could prevent global spread of antimalarial resistance. Eliminating malaria rapidly requires targeting the reservoir of asymptomatic parasite carriers. This pilot trial aimed to evaluate the acceptability, safety, feasibility and effectiveness of mass-drug administration (MDA) in reducing malaria in four v… Show more

“…This has been observed also in other MDA trials [30], and is best explained by the hypnozoite stage of P. vivax causing relapses of the primary infection. Hypnozoites are not affected by the MDA regimen and require a prolonged course of primaquine.…”

“…A similar impact of the same MDA regimen as an addition to establishment of a network of malaria posts has recently been demonstrated in Myanmar [30]. MDA efforts elsewhere have interrupted malaria transmission permanently on Pacific islands [33].…”

“…Combining ivermectin with antimalarial MDA would be a novel way to reduce transmission of Plasmodium parasites, as it targets the malaria parasite in the host and mosquito vector. MDA coverage rarely reaches 100% of the eligible population, adherence may be poor, and malaria asymptomatic or healthy people may not perceive a direct personal benefit from participation in ACT‐based MDAs . However, experience in Africa shows that people do recognize direct personal benefits from participating in ivermectin MDAs because the drug affects several neglected tropical diseases, which are common in malaria‐endemic areas .…”

“…One example is the malaria elimination efforts in the Greater Mekong Subregion. These focus on P. falciparum, due to the urgency engendered by the emergence of artemisinin resistance [57], but had no impact on vivax malaria since radical cure was not given to patients who presented with it [58].…”
