Safety and efficacy of a novel robotic transcranial doppler system in subarachnoid hemorrhage

Abstract: Delayed cerebral ischemia (DCI) secondary to vasospasm is a determinate of outcomes following non-traumatic subarachnoid hemorrhage (SAH). SAH patients are monitored using transcranial doppler (TCD) to measure cerebral blood flow velocities (CBFv). However, the accuracy and precision of manually acquired TCD can be operator dependent. The NovaGuide robotic TCD system attempts to standardize acquisition. This investigation evaluated the safety and efficacy of the NovaGuide system in SAH patients in a Neuro ICU.… Show more

“…Thorpe et al used TCD to recognize CBF waveform features (quantified as the velocity curvature index and velocity asymmetry index) that achieved good diagnostic accuracy for LVO. 43 Because the required technical expertise may limit broad implementation in the prehospital setting, Neural Analytics developed an autonomous system that obviates the need for an expert sonographer (NovaGuide™ TCD, Neural Analytics Inc, United States), 44 but its potential role as in detecting LVOs in clinical practice remains unclear.…”

“…Emerging incorporation of point-of-care ultrasound in critical care now incorporating TCD may increase availability and expertise in its bedside application. Automated robotic TCD machines have the potential to allow continuous monitoring, allowing widespread use (30)(31)(32).…”

“…However, advances in robotic TCD technology may overcome the latter. 23,24 TCD is routinely used in neurocritical care to monitor for vasospasm in SAH patients and assess individual treatment responses. TCD measurements can guide the timing and the need for repeat endovascular interventions.…”
