Safety and Outcomes of Short-Term Multiple Femoral Venous Sheath Placement in Cardiac Electrophysiological Study and Radiofrequency Catheter Ablation

Abstract: SUMMARYMultiple intracardiac catheters are often necessary for electrophysiological study (EPS) and radiofrequency (RF) ablation therapy. Therefore, multiple venous sheath placement in one femoral vein is always required for multiple intracardiac catheter insertion. The vascular complications incurred by placement of multiple sheaths have not been fully studied. We utilized duplex ultrasonography to assess the femoral veins before and after the procedure. This study consisted of 52 patients (68 femoral veins) … Show more

“…The risk remains small (0.6%) even if a therapeutic manoeuvre, such as a radiofrequency ablation, is added [3]. It has been reported, however, that the incidence of the formation of non-occlusive deep vein thrombosis at the puncture site is up to 17.6% [4]. The guidelines for clinical intracardiac procedures do not mention specific measures for prevention of venous thromboembolism [5], but generally it is agreed that some form of thromboprophylaxis should be administered.…”

“…But, none of these patients with this complication experienced symptomatic PE. 8 In our patient, three sheaths were inserted into the right femoral vein, ipsilateral to an 8-Frech femoral artery sheath. A long-duration (6 hours) hemostasis procedure for the femoral artery sheath may also compress the femoral vein simultaneously, leading to venous stasis.…”

“…Color Doppler ultrasonography has been used to evaluate peripheral vessels and, in preliminary studies, to localize the cephalic vein in assisting device implantation 17–20 . In the current study, we prospectively utilize color Doppler ultrasonography to identify the characteristics and abnormalities of the cephalic vein.…”
