Salvage esophagectomy: safe therapeutic strategy?

Abstract: The objectives of this review were to assess both the short-and long-term clinical outcomes in patients managed with definitive chemoradiotherapy, and salvage esophagectomy subsequently in comparison to those neoadjuvant chemoradiotherapy followed by planned esophagectomy (NCRS) for esophageal cancer from published literature. Eleven studies comprising 1,906 patients were included, 563 in the salvage group and 1,343 in the NCRS group. Pooled analysis showed no significant difference between salvage and NCRS gr… Show more

“…On the other hand, neoadjuvant CRT is accepted as the standard treatment for locally advanced esophageal cancer in Western countries [ 23 , 24 ]. However, neoadjuvant CRT is found to be associated with increased mortality and morbidity after surgery due to irradiation toxicity [ 25 , 26 ]. Therefore, our strategy in this case might reduce the therapeutic toxicity and lead to no major complications, consequently.…”

“…Salvage esophagectomy after previous definitive chemoradiotherapy is a well-established treatment strategy [44]. Any role of surgery in stage IV disease can be disputed.…”

“…Despite evidence for technical feasibility in expert hands, the efficacy of salvage surgical resection is still debated. 36 Head-to-head comparison between salvage ER and salvage esophagectomy for local recurrence is not easily possible, because highly selected patient groups are included in both groups. Salvage ER may well have a better safety profile compared with salvage surgery for local residual or recurrent lesions after definitive CRT.…”
