Salvage extracorporeal membrane oxygenation in induction-associated acute respiratory distress syndrome in acute leukemia patients: A case series

Abstract: Currently, the use of extracorporeal membrane oxygenation in patients with hematologic malignancies who develop severe acute respiratory distress syndrome remains controversial. Although extracorporeal membrane oxygenation in post-allogeneic hematopoietic stem cell transplant is associated with poorer outcomes, our data suggest that salvage extracorporeal membrane oxygenation support is a viable option to manage moderate to severe acute respiratory distress syndrome while completing therapeutic chemotherapy an… Show more

“…A few studies have been published on outcomes of ECMO in patients with malignant diseases. According to recent studies, the median overall survival rate was still low in adult patients with hematologic malignancies who developed ARDS receiving ECMO ( 9 , 10 , 15 ). More recently, the largest study reported the outcome of 297 adult patients with cancer with severe ARDS, and the 60-day survival rate was 26.8% (95% CI 22.1–32.4%).…”

“…ECMO can provide cardiac and pulmonary support in critically ill patients with cardiogenic shock or acute respiratory failure[ 5 ]. The use of ECMO in cancer patients who develop severe acute respiratory distress syndrome (ARDS) remains controversial[ 6 ]. However, several studies have reported the successful use of ECMO in patients who develop respiratory failure after chemotherapy for hematologic malignancies[ 3 , 4 , 6 , 7 ].…”

However, several studies have reported the successful use of ECMO in patients who develop respiratory failure after chemotherapy for hematologic malignancies[ 3 , 4 , 6 , 7 ]. Chung et al [ 8 ] reported that under ECMO support, the patient underwent bronchial stent implantation and was successfully weaned off ECMO.…”

“…Venovenous extracorporeal membrane oxygenation (VV ECMO) is a mechanical-assisted therapy for the extracorporeal gas exchange in patients with severe respiratory failure, which oxygenates and removes carbon dioxide from the blood [ 8 , 9 ]. VV ECMO has been reported as successful in limited case studies for acute pneumonia with hematologic malignancies, including in S. maltophilia hemorrhagic pneumonia associated with hematologic malignancies [ 10 , 11 ].…”
