Salvage preoperative embolization of an infratemporal solitary fibrous tumor

Abstract: Rationale:Head and Neck Solitary fibrous tumors (SFT) are very rare. They could be misdiagnosed as hemangiopericytomas (HPC).Patient concerns:We report a 60 y o lady presenting with sinonasal mass, causing recurrent profuse bleeding.Diagnoses:Hemangioperocytomas versus SFT were among the differentials, according to Radiological studies. Upon Biopsy, the diagnosis of SFT has been adopted.Interventions:Salvage pre-operative embolization resulted in bleeding control, bridging the patient to surgery.Outcomes:Post-… Show more

“…Additionally, the resulting necrotic softening of the tumor can simplify the resection [ 26 ]. The use of spherical particles, polyvinyl alcohol (PVA) and Onyx (ethylene vinyl alcohol copolymer dissolved in dimethyl sulfoxide and micronized tantalum powder; EV3, Irvine-CA) has been described in transarterial embolization of the head and neck SFTs [ 20 , 27 ]. Just like PVA, microspheres (including Embozene that was used in our case) block the vessel inducing inflammatory reaction, angionecrosis and fibrosis.…”

“…The first choice of treatment is surgical resection, and preoperative embolization is effective for highly irrigated tumors to avoid intraoperative bleeding and incomplete removal, which leads to high local recurrence rates (12). SFT surgery combined with preoperative vascular embolism has already been reported (13)(14)(15)(16), but not in the tongue. Intraoperative and postoperative bleeding can also lead to airway problems.…”

“…Some reported that local recurrence seldom occurred if gross total resection was achieved [9 , 10] . However, once intratumor resection occurred, a high rate of local recurrence was observed, and the tumor will possibly be dedifferentiated [11 , 12] . Indeed, our cases demonstrated that the tumors were infiltrating into the bilateral multifidus, and it would be difficult to perform operation without intratumor resection by ordinal laminectomy.…”
