“Sandal strap” trauma and atherosclerosis are dual pathologies leading to bilateral true aneurysms of the dorsalis pedis arteries

Abstract: True aneurysm of the dorsalis pedis artery is rare. To our best knowledge, only 19 cases have been reported in the literature with one case of bilateral involvement. We describe an unusual case of simultaneous, bilateral true aneurysms of the dorsalis pedis arteries due to chronic mild trauma from dorsally positioned sandal straps with secondary atherosclerotic change. This is the first such case reported. Symptomatic aneurysms are at risk for thrombosis or embolization, and surgical management is recommended.… Show more

“…5 It is worth noting that repeated minor injuries, such as those caused by ill-fitting shoes, may promote the development of DPA aneurysms. 6 As in my case report, the most frequently described clinical manifestations of aneurysms of the dorsal artery of the foot are a pulsating mass, painful or painless, sometimes associated with itching and discomfort. 7,8 Aragao et al published a review of the literature in 2017, highlighting 24 reported true aneurysms of the dorsalis pedis.…”

“…Primary dorsalis pedis reconstruction is not well described in the current literature. There are several case reports that describe aneurysmal disease after a traumatic crush injury, arterial access complications, and even attributed to repetitive footwear trauma [12][13][14][15][16]. Long-term patency conclusions from this data are difficult to infer; however, saphenous vein bypass to the dorsalis pedis in atherosclerotic disease has been described.…”

“…Trauma is the most common cause of aneurysm in this site however, it should be noted that patients may not recall the acute event as symptoms may only present once the patient experiences compression of local structures or embolic/rupture events 3. In comparison to an acute traumatic event, there are cases highlighting the role of repetitive low impact trauma causing atherosclerotic change and aneurysmal degeneration in patients otherwise not predisposed to atherosclerosis—examples include tight fitting shoes in patients with congenitally high arches as reported by Sonntag et al 4…”
