SARS-CoV-2 vertical transmission supports innate fetal protection: A narrative review

Abstract: Prenatal infections that have been exhaustively studied help frame the current Severe Acute Respiratory Syndrome related coronavirus-2 (SARS-CoV-2) pandemic, with the caveat that asymptomatic SARS-CoV-2 infected patients are not tested, while those symptomatic are delivered and/or treated with drug(s) available on-site. Thus, management and therapy are still heterogeneous. SARS-CoV-2 induced respiratory infection remains mostly local, unless severe, which lessens transplacental vertical transmission (VT). Vacc… Show more

“…However, the current reality is that even in those integrated settings, effective care may be limited due to a high volume of patients. The level of care and the need for integration has been previously addressed by the authors and others 2–5 . At present, the shortage of trained, available personnel is still significant in LMICs, which contributes to disparate outcomes among clinical facilities.…”

“…When a patient with COVID-19 requires a cesarean there is added complexity for staff, patient, newborn, and throughout the duration of hospital stay, and particularly for the procedure itself, delivery, and the delayed recovery period. 4 The monetary incentive for surgeons to perform a cesarean or opt for vaginal delivery should be part of a facility's quality control system and the difference in compensation for cesarean versus vaginal delivery should be minimized unless the procedure and the complexity is elevated.…”

“…Fortunately, infection rates continue to decrease due to utilization of vaccination, although significant improvement has not been seen in LMICs. When a patient with COVID‐19 requires a cesarean there is added complexity for staff, patient, newborn, and throughout the duration of hospital stay, and particularly for the procedure itself, delivery, and the delayed recovery period 4 …”
