SAT-140 A Drug-Drug Interaction Trial of Oral Semaglutide with Levothyroxine and Multiple Coadministered Tablets

Abstract: Introduction: Oral semaglutide contains the GLP-1 analog semaglutide co-formulated with the absorption enhancer, sodium N-(8-[2-hydroxybenzoyl] amino) caprylate (SNAC). Aims: This trial investigated (A) the effect of oral semaglutide and SNAC alone on the pharmacokinetics (PK) of levothyroxine (thyroxine), a frequently used oral medication with similar dosing instructions to oral semaglutide, and (B) the effect of multiple co-administered tablets on oral semaglutide PK. … Show more

“…4,9 Drugs that have a narrow therapeutic index may require increased clinical or laboratory monitoring when used in combination with oral semaglutide. 4,9,10 In one study, levothyroxine exposure increased by 33% (90% CI: 125-142) when administered with oral semaglutide. 10 There is currently no guidance on recommendations for administering these medications in practice, and this may present a challenge for patients currently taking levothyroxine because it has similar administration considerations.…”

Patients may have to administer levothyroxine on an alternative schedule, such as at night, 3 to 4 hours after the last meal.…”

“…• As food and other tablets can impact the absorption of oral semaglutide [70,78], the patient should be instructed to take oral semaglutide on an empty stomach after they wake up [9]. • The patient should swallow the tablet whole (not split, crushed, or chewed) with a sip of plain water (up to 4 floz/120 mL) and then wait at least 30 minutes before eating, drinking beverages, or taking other oral medications [9].…”

“…An example of such an agent is levothyroxine, which is used as a thyroid hormone replacement therapy and is recommended to be taken on an empty stomach before breakfast [77]. When a single dose of levothyroxine 600 µg was co-administered with oral semaglutide at steady-state in a study in healthy volunteers, thyroxine exposure increased by 33%, potentially due to a delay in gastric emptying caused by oral semaglutide [9,78]. Patients taking concomitant levothyroxine should therefore be advised to comply with the dosing conditions for oral semaglutide [9].…”
