Scaling-up primary health care-based prevention and management of heavy drinking at the municipal level in middle-income countries in Latin America: Background and protocol for a three-country quasi-experimental study

Abstract: Background: While primary health care (PHC)-based prevention and management of heavy drinking is clinically effective and cost-effective, it remains poorly implemented in routine practice. Systematic reviews and multi-country studies have demonstrated the ability of training and support programmes to increase PHC-based screening and brief advice activity to reduce heavy drinking. However, gains have been only modest and short term at best. WHO studies have concluded that a more effective uptake could be achiev… Show more

“…The clinical package, tailored for local use, comprised measurement instruments, patient information and advice material, and provider guidance material, with the differences between the standard (more intense) and less intense clinical materials described in the published protocol 35 . The less intense version was a simplified version of standard UK-based materials 49 , deliverable in a reduced period of time, noting evidence that the duration of brief advice has little impact on outcome 7 , 50 , 51 .…”

Currently, great amount of research is focused on implementation, where attention has been drawn to the need of also addressing the context where implementation of alcohol intervention takes place (195).…”

“…Since this study's main goal was one of quality improvement in the participating primary health-care facilities without collecting any identifying patient information, it was considered to be part of routine care by participating institutions, except for the specialized addiction care centre, where it underwent ethical review (this is similar to other implementation studies for screening and brief advice/interventions in other countries 22 ). Even though the study was one of quality control, we asked for verbal consent from patients willing to participate.…”
