Little is known about the arachnoid cyst and there have been few reports of it accompanying psychiatric disturbances. A 57-year-old patient was admitted due to sudden headache, auditory hallucination, and delusion of persecution. An arachnoid cyst was found in the anteromedial aspect of middle cranial fossa on his magnetic resonance image. This was his first psychotic episode and he was also negative to other clinical evaluations including endocrine system. His psychotic symtoms were suspected to be induced by the arachnoid cyst and it was well controlled by low-dose risperidone administration. He left hospital free from psychotic symptoms on 14th hospital day.

ÖZET:Kırkdört yıllık iyileşme dönemi sonrası tekrarlayan mani: Bir olgu sunumu İki uçlu bozukluk, geçen yıllar ve artan dönem sayısı ile aradaki iyilik dönemlerinin ve hecmeler arası sürenin kısaldığına inanılan ilerleyici bir hastalıktır. Bununla birlikte birçok iki uçlu hastada hastalık istikrasız ve tahmin edilemez bir seyir izlemektedir. Bazı iki uçlu hastalar koruyucu tedavi almamalarına karşın yeni bir atak yaşamakta ya da dönemler arası süre onlarca yılı bulabilmektedir. Biz bu yazıda, koruyucu tedavi olmaksızın 44 yıl düzelmede olan bir mani vakasını sunmayı ve koruyucu tedavinin gerekliliğini ve süresini tartışmayı amaçladık. However, in many bipolar patients an erratic and unpredictable course of illness may occur. Experienced clinicians are familiar with some patients, whom have not suffered from a relapse of illness for years despite lack of prophylactic treatment. In this report we present a case of mania with 44 years of remission without prophylactic treatment and discuss the preventive therapy in bipolar disorder.
