Schmorl’s Node Developing in the Lumbar Vertebra Affected With Metastatic Carcinoma: Correlation Magnetic Resonance Imaging With Histological Findings

Abstract: MR features consisting of two different signal patterns are characteristic for Schmorl's node with a neoplastic background. The conditions should be distinguished from Schmorl's nodes in an acute phase, which show similar MR features.

“…[17][18][19][20][21][22][23][24][25] The medical history of the patients is often omitted, but it may include trauma (fall from a height), 26 degenerative disorders (spondylolisthesis, degenerative disc disease, scoliosis), 21,22,27 congenital disorders (spondylolysis and Scheuermann's kyphosis), 21,27,28 or neoplasia (plasmacytoma). 29,30 With a few exceptions, a traumatic mechanism occurs before the formation of an acute SN. Of the 44 published cases of an acute SN, 41 patients (93%) reported some kind of injury prior to the onset of symptoms.…”

“…21,22 En este punto, sugerimos realizar una biopsia durante el procedimiento, antes de inyectar el cemento, dados los datos clínicos de pérdida de peso y la similitud que existe en el patrón de señal de la RM entre un NS agudo y con un fondo neoplásico. 23 Es muy raro que la primera manifestación de un carcinoma pancreático sea una metástasis vertebral. Las localizaciones más frecuentes de metástasis en el cáncer de páncreas son el hígado y el peritoneo.…”

“…In addition to SNa associated with low BMD ( 22 ) and SNa associated with acute high energy trauma ( 24 , 25 , 38 , 43 ), various other etiology with weakening of the endplate, including infectious spondylodiscitis or other inflammatory spine diseases such as ankylosing spondylitis, may also cause SNa ( 23 , 44 ) ( Figure 23 ). Tumorous changes of a vertebra can also increase the fragility of an endplate, and lead to disc materials herniation through endplate into the vertebral spongy bone and form SNa ( 45 , 46 ). Peng et al ( 47 ) reported that SNa can be the end result of ischaemic necrosis beneath the cartilaginous endplate.…”
