The Protective Effects of Danggui-Baizhu-Tang on High-Fat Diet-Induced Obesity in Mice by Activating Thermogenesis

Danggui-Baizhu-Tang (DBT), a traditional Chinese medicine decoction, was used for decreasing serum TG and TC remarkably. However, effect of weight control and action mechanism remains confused. In this study, to evaluate the anti-obesity effects, different gradient concentration of DBT (0.59, 1.17 g/kg) or Orlistat (Orl, 15.6 mg/kg; positive control) were administrated by gavage for 8 weeks in C57BL/6J mice which were pretreated with chow or high fat diet (HFD) for 3 months. After administration, significant decrease of body weight and food utilization was observed. It was indicated that concentration of triacylglycerol (TG), total cholesterol (TC), alanine aminotransferase (ALT), aspartate aminotransferase (AST) in serum were reduced strikingly, as well as accumulation of lipid droplets in liver. Meanwhile, DBT treatment could also decrease weight of white adipose tissue (WAT) and size of adipocytes, whereas increase weight of brown adipose tissue (BAT) in mice. Moreover, it was revealed that DBT could elevate rectal temperature by raising expression of uncoupling protein-1 (UCP1) and peroxisome proliferator-activated receptor-gamma coactivator-1alpha (PGC-1α), which were attributed to phosphorylation of AMP-activated protein kinase (AMPK). Furthermore, TNF-α and IL-6, obesity-related inflammatory cytokines, were decreased. In conclusion, DBT could stimulate phosphorylation of AMPK to raise expression of UCP1 and PGC-1α, and activate thermogenesis to prevent obesity.

Aim: Oligohydramnios contributes to a negative fetal prognosis. Management of oligohydramnios in obstetric clinical practice is important. We assessed the clinical efficacy of Gorei-San for the treatment of oligohydramnios. Methods: Single deepest pocket (SDP) changes before and after treatment for oligohydramnios (SDP decrease to less than 3 cm) were examined. A total of 51 pregnant mothers who were experiencing oligohydramnios were enrolled in the study. The changes in SDP were compared between 27 women treated with Gorei-San (7.5 g /day) and 24 untreated women. The first measurements of SDP in both groups were obtained when the SDP had decreased to less than 3 cm (start point: SP), and the second measurements were obtained at the next regular examination (end point: EP). Results: The average SDP significantly increased in women treated with Gorei-San from the SP (2.37 ± 0.41 cm) (mean ± SD) to the EP (3.15 ± 0.62 cm) (mean ± SD) (P < 0.001). In contrast, there were no significant changes in the untreated women from the SP (2.5 ± 0.15 cm) (median ± quartile) (non-normally distributed) to the EP (2.63 ± 0.68 cm) (mean ± SD) (P = 0.59). Conclusions: Gorei-San was helpful in the treatment of oligohydramnios. Although further studies are needed to confirm our findings, we believe that this new application of Gorei-San for the treatment of oligohydramnios is a clinically applicable strategy.

Successful treatment of refractory edema with traditional herbal medicine

Rationale:Refractory edema is characterized by persistent swelling which does not react to diuretic use and sodium restriction. Traditional herbal medicine, Gwack Rhyung Tang and Chunggan extract effectively treated refractory lower limb edema caused by cirrhosis and improved liver function.Patient concerns:A 64-year-old male patient with a history of hypertension, diabetes mellitus, hepatic encephalopathy, and cellulitis presented lower limb edema which did not react to diuretics for more than 7 months.Diagnoses:Refractory edema caused by cirrhosis.Interventions:The patient was treated for 25 days using Gwack Rhyung Tang and Chunggan extract.Outcomes:Loss of body weight, decrease in circumferences of both lower limb and improvement of liver function biochemistry results are checked. There was no recurrence or aggravation of the condition up to 3 weeks of follow-up periods.Lessons:Traditional herbal medicine can be an effective alternative for refractory edema due to cirrhosis with improving liver function.
