Scoping Review: Peran Rasio Neutrofil-Limfosit sebagai Biomarker pada Pasien HIV dengan Komorbid atau Tanpa Komorbid

Abstract

Abstract. Human Immunodeficiency Virus (HIV) is a virus that damages the function of immune cells. Laboratory test that have become the gold standard for monitoring the progression of HIV stages are the examination of the CD4 T lymphocyte count. These test are expensive and trained technicians are not always available in some parts of Indonesia, making it impractical to use them for routine monitoring. The limitations of this examination cause the need for other laboratory tests that can be used as an alternative to the CD4 T lymphocyte count. The aim of this study was to determine the role of the neutrophil-lymphocyte ratio as a biomarker in comorbid and non-comorbid HIV patients. This study is a scoping review study, samples from international and national journals relating to the role of the neutrophil-lymphocyte ratio as a biomarker in HIV patients with comorbid or non-comorbid HIV. The databases used in this study were PubMed, Springer Link, and ProQuest with 1,170 articles. Screening results on articles that match the inclusion criteria were 714 articles. There were 699 articles that entered the exclusion criteria. The results of the eligibility criteria were obtained as many as 10 articles and then analyzed. The study was conducted during September–December 2020. The conclusion of this study is that the neutrophil-lymphocyte ratio in HIV-comorbid patients acts as a prognostic biomarker. Neutrophil-lymphocyte ratio in HIV patients without comorbids acts as a predictive biomarker and risk biomarker.

 Abstrak. Human Immunodeficiency Virus (HIV) merupakan virus yang merusak fungsi sel-sel kekebalan tubuh. Pemeriksaan laboratorium yang menjadi baku emas untuk memantau perkembangan stadium HIV adalah pemeriksaan jumlah limfosit T CD4. Pemeriksaan tersebut mahal dan teknisi yang terlatih tidak selalu tersedia di beberapa daerah di Indonesia sehingga kurang praktis digunakan untuk pemantauan secara rutin. Keterbatasan pemeriksaan tersebut menyebabkan diperlukan adanya pemeriksaan laboratorium lain yang dapat dijadikan alternatif selain jumlah limfosit T CD4. Tujuan penelitian ini adalah untuk mengetahui peran rasio neutrofil-limfosit sebagai biomarker pada pasien HIV dengan komorbid atau tanpa komorbid. Penelitian ini merupakan penelitian scoping review, sampel berupa artikel dari jurnal internasional dan nasional yang berkaitan dengan peran rasio neutrofil-limfosit sebagai biomarker pada pasien HIV dengan komorbid atau tanpa komorbid. Database yang digunakan dalam penelitian ini adalah PubMed, Springer Link, dan ProQuest dengan jumlah artikel yang di dapat sebanyak 1.170 artikel. Hasil skrining pada artikel yang sesuai dengan kriteria inklusi sebanyak 714 artikel. Artikel yang masuk kriteria eksklusi sebanyak 699 artikel. Hasil uji kelayakan didapatkan sebanyak 10 artikel dan kemudian dilakukan analisis. Penelitian dilakukan pada periode September–Desember 2020. Kesimpulan penelitian ini adalah bahwa rasio neutrofil-limfosit pada pasien HIV dengan komorbid berperan sebagai prognostic biomarker. Rasio neutrofil-limfosit pada pasien HIV tanpa komorbid berperan sebagai predictive biomarker dan risk biomarker.
