Objective: The global spread of COVID-19 has caused great psychological stress in health care workers (HCWs). This study aims to assess the prevalence and associated factors of posttraumatic stress symptoms (PTSS) and stigma among health care workers (HCWs) who are involved in treating COVID-19 patients. This study was conducted as part of studies related to assessing the mental health status of HCWs in Iran during the COVID-19 pandemic. Method: Overall, in this cross sectional study, 894 HCWs working in 9 general hospitals in Alborz province, Iran, in the field of diagnostic and treatment care of patients with COVID-19 were selected using multistage sampling method. PTSS and its subscales, including intrusion, avoidance, and hyper vigilance were obtained based on the Posttraumatic Stress Disorder-8 Item validated questionnaire. Information on Stigma's perception among HCWs was also obtained based on a questionnaire adopted from the HIV Stigma Scale. Predictors of PTSS and Stigma's perception among HCWs were assessed using multivariate logistic regression analysis. Results: Overall, 71.4% of the participants were women and 46.9% were front line staff. The prevalence of intrusion, avoidance, and hyper vigilance symptoms was 44.2% (95% CI: 40.8-47.6), 31. 8% (95% CI: 28.8-35.0), and 37.7% (95% CI: 34.5-41.0), respectively. A strong and positive significant correlation was found between stigma score with PTSD total score (coefficient: 0.83) and its components. In multivariate logistic regression model, female gender was associated with intrusion (OR: 1.46, 95% CI: 1.03-2.06) and avoidance (OR: 1.66, 95% CI: 1.147-2.417) and working in frontline increased the odds of intrusion (OR: 1.45, 95% CI: 1.06-1.97) and hyper vigilance (OR: 1.41, 95% CI: 1.03-1.91). Conclusion: The prevalence of PTSS is high among HCWs during COVID-19 pandemic and it is associated with some demographic characteristics of HCWs. This situation should be considered by health policymakers so that while trying to control the disease, the mental health status of this group of personnel should be given much attention.

Resumo O objetivo deste estudo é identificar a prevalência de ansiedade em profissionais de saúde durante a pandemia da COVID-19. Trata-se de revisão sistemática de estudos publicados em qualquer idioma em 2020. Foi realizada busca nas bases de dados Embase, LILACS e PubMed utilizando os descritores anxiety, COVID-19, health workers, e sinônimos. A estimativa da prevalência geral de ansiedade com intervalo de confiança de 95% foi calculada utilizando o modelo de efeitos aleatórios. Dos 861 registros identificados, 36 artigos foram incluídos na revisão sistemática e 35 na metanálise. A prevalência geral de ansiedade foi de 35% (IC95%: 29-40). Foi identificado maior risco de ansiedade nas mulheres em relação aos homens (Odds Ratio: 1.64 [IC95%: 1,47-1,84]), e nos enfermeiros, na comparação com médicos (Odds Ratio: 1.19 [IC95%: 1,07-1,33]). Atuar na linha de frente no combate a COVID-19, estar infectado com coronavírus e apresentar doenças crônicas também foram fatores associados com maior risco de ansiedade. Observa-se alta prevalência de ansiedade entre profissionais de saúde, com maior risco entre mulheres e enfermeiros. Há necessidade de medidas que visem sua prevenção, bem como o fornecimento de tratamento precoce e adequado aos com ansiedade moderada e grave.

Background Covid‐19 has caused many complications for both the infected and those in need of medical care. This may be due to infection‐related prognosis worsening or the patients’ avoidance of referring to the hospital for fear of contracting the infection. The decline of acute referral to the ER of many significant conditions with severe results on both the well‐being and life‐expectancy is a serious concern. To address these concerns, we designed this study to evaluate the recent pandemic's impact on “in‐hospital mortality” caused by neurological disorders pre and postpandemic. Methods and material The inclusion criteria were any acute neurological condition and the consent of the patients eligible for our study. The definitions of all assessed conditions and the comorbidities are ICD‐10 based. Hypertension and diabetes mellitus, due to their high prevalence, were evaluated separately from other internal comorbidities. The total number of the enrolled patients was 1742, 671 of whom had been during the COVID‐19 pandemic, and 1071 had attended the ER during the prepandemic era. Results The overall mortality was significantly higher during the pandemic, and the covid infected had suffered higher mortality rates. ( p ‐value < 0.05) Conclusion During the pandemic, those with minor strokes and other nonlife‐threatening ailments had avoided hospital care leading to a significantly higher rate of critical conditions. Increased incidence of strokes and ICHs during the pandemic, too, caused significant increased in‐hospital mortality.
