HLA and disease: guilt by association

Abstract: It is now over forty years since the first associations between particular HLA antigens and disease susceptibility were described, and the identification of large numbers HLA-associated diseases parallels our increased understanding of the genetic complexity of the HLA system and its extensive polymorphism. However, surprisingly and frustratingly, clear identification of the underlying mechanisms resulting in a causative role for HLA polymorphism in the molecular immunopathogenesis of individual HLA-associated… Show more

“…Many of the immunological diseases that occur with AIH, namely autoimmune thyroiditis, RA, Sjogren's syndrome and IBD have been associated with this extended haplotype [30][31][32][33][34][35][36] . The clustering of the CAIDs in AIH may suggest the shared genetic and immunological susceptibility [37,38] . In a series of 122 patients with type 1 AIH, HLA-DR4 was significantly more common in those with CAIDs than in those without these features (62 vs. 33%, p = 0.01) [23] .…”

“…These associations involve malignant as well as autoimmune diseases, including the well-known association between HLA-B27 and ankylosing spondylitis. Over time, several hundred diseases have been reported to occur more frequently in individuals with particular HLA genotypes (2). These diseases include a broad spectrum of immune-mediated diseases involving all major organ systems, certain malignancies, infectious diseases and, more recently, adverse reactions to particular drugs (2).…”

“…Alternatively, HLA may play a causative role via influence on the T-cell repertoire, including T regulatory cells, resulting in potential autoreactivity. Finally, the associated HLA polymorphism may play no direct role, merely acting as a marker [8,[24][25][26]. Studies in other populations gave varying results.…”
