NAFLDin type 2 diabetes mellitus: Still many challenging questions

Simona Cernea,
Itamar Raz

Abstract: Summary Epidemiologic data consistently show that in patients with type 2 diabetes (T2DM) the prevalence of non‐alcoholic fatty liver disease (NAFLD), including advanced fibrosis, is double compared to the general population, and it associates with high risk of liver‐related morbidity (advanced fibrosis, hepatocellular carcinoma) and mortality, but also with other systemic consequences, such as cardiovascular (CV) disease, chronic kidney disease, and overall mortality. There are still many answers that need to… Show more

“…NAFLD can progress to hepatic inflammation and fibrosis (non-alcoholic steatohepatitis, NASH). The prevalence of NAFLD and its progression to NASH is about double in patients with T2DM than in non-diabetic individuals (Cernea and Raz, 2020).…”

“…NAFLD affects various regulatory pathways and extra‐hepatic organs, and increases the risk of T2DM and CVD, CKD. However, a direct causal relationship has not been clearly delineated, nor the prognostic value of NAFLD for risk stratification, nor if reversing NAFLD also reverses CVD, CKD, or T2DM 8,9 …”

“…But knowledge of advanced fibrosis may spur more intensive weight loss efforts, which can lead to reductions in fibrosis and improved outcomes 61–63 . Application of FIB‐4 or NFS scoring in primary care, especially in patients with type 2 diabetes, could prompt further testing or diagnostic assignment in those without a known liver disease diagnosis 22 …”

“…Despite the close relationship to these primary care conditions, NAFLD is underdiagnosed in primary care 11–16 . Broad screening recommendations for at‐risk patients are conflicting, but multiple guidelines address the importance of NAFLD detection in patients with abnormal liver tests (with NAFLD the most common cause of aminotransferase abnormality) and those with diabetes 1,5,17–22 …”
