Screening for frailty among older patients with cancer using blood biomarkers of inflammation

Abstract: Frailty and GPS two are strongly associated, and GPS two is a significant prognostic factor in frail, older patients with cancer. The inflammatory biomarkers investigated are not suitable screening tools for frailty.

“…With respect to survival in cancer patients, few studies have directly compared frailty and inflammation ( 33 ). Our findings suggest that the co-occurrence of inflammation and frailty is associated with a high mortality risk, which is consistent with results reported in existing literature ( 34 ). Additionally, in our study, the hazard ratios of frailty with inflammation were higher for tumors with relatively long-life expectancies than tumors with higher lethality [HR (95%CI): 2.02 (1.52-2.69) vs. 1.69 (1.36-2.10)].…”

“…Eight studies measured frailty as a deficit accumulation index or geriatric vulnerability scores using the Leuven Oncogeriatric Frailty Score ( 27 , 32 , 34 ), Balducci criteria ( 27 , 30 , 32 , 34 ), Flemish Triage Risk Screening Tool (fTRST) ( 27 ), G8 ( 27 ), geriatric assessment domains ( 30 , 31 ), the geriatric vulnerability score ( 23 ), the Edmonton frailty scale ( 35 ), and the Carolina frailty index ( 37 ). One study ( 23 ) used geriatric assessment domains that also included hypoalbuminemia and lymphopenia as two additional vulnerabilities calculated into the total geriatric vulnerability score.…”

“…In patients with mCRC, high baseline CRP is associated with shorter survival [ 21 ]. Moreover, CRP is known as a biomarker of frailty, is linked to sarcopenia, functional decline, increased risk of toxicity, decreased QoL, and higher mortality in older adults with different types of malignancies [ 15 , 22 , 23 , 24 , 25 , 26 ].…”
