Secondary hyperparathyroidism prevalence and prognostic role in elderly males with heart failure

Abstract: SHPT was highly prevalent in elderly males with HF and was associated with impaired survival. HF patients with SHPT had more severe disease compared to the patients with normal serum PTH. Determination of serum PTH levels provided additional value to NT-proBNP for risk stratification in these patients.

“…Patients with HF had higher rates of PTH, irrespectively of its etiology what remains in line with previously published studies [ 23 27 ]. PTH concentration in HF correlates with disease severity, expressed by decreased LVEF, increased EDV, and functional NYHA class.…”

“…VDD combined with raised PTH represents a vicious cycle of mechanisms on one hand leading to HF and in the other hand worsening the prognosis of HF. It is worth to mention that myocardial fibrosis and hypertrophy, excessive adrenergic stimulation, calcium cell overload, oxidative stress enhancement, and FGF-23 formation have huge impact on impaired survival [ 23 ].…”

“…Patients with insufficiency cardiac and secondary hyperparathyroidism had more severe myocardial dysfunction compared with those with normal serum PTH, and determination of serum PTH levels provided additional value to NT‐proBNP for risk stratification in these patients 48. In a recent prospective study, correcting vitamin D deficiency in HD patients in associated with better secondary hyperparathyroidism control with lower doses of vitamin D analogues, as well as an improvement in inflammatory status 49…”

“…These studies suggest severe hypocalcemia associated with rickets in infants and hypoparathyroidism as causes of DCM, cardiac remodeling, HF, and impaired cardiac function. Improvement in clinical symptoms after vitamin D and calcium supplementation for the treatment of hypovitaminosis and hypocalcemia signifies the importance and protective role of vitamin D in cardiomyopathy [9–10, 3439] (Table 1). Gruson et al [40] suggested that serum 1,25(OH) 2 D 3 and its ratio to parathyroid hormone (PTH) (1–84) strongly and independently predict cardiovascular mortality in chronic HF.…”
