Secondary prevention after stroke/transient ischemic attack: A randomized audit and feedback trial

Abstract: Objectives:The clinical benefits of use of secondary preventive pharmacotherapy in ischemic stroke/TIA have been previously demonstrated. A potential target for facilitating the use of recommended medications is primary care physicians. Therefore, we carried out an audit and feedback intervention aimed at primary care centers. The aim was to improve the use of secondary preventive stroke medications and diagnosis recording in ischemic stroke/TIA. Materials and Methods:The intervention consisted of structured, h… Show more

“…Evidence-based medicine and guidelines have improved the quality of health care, but still suboptimal adherence to care guidelines is a common worldwide problem seen not only with asthma 2-5 and chronic obstructive pulmonary disease [52][53][54] but also with other common chronic conditions, such as cardiovascular diseases and diabetes [55][56][57][58][59] . GPs generally deal with multimorbid patients.…”

“…Similar findings have been presented in earlier Swedish and international studies. [37][38][39][40] An interaction between AF and statin prescriptions, suggests that women getting anticoagulation, are less likely to be prescribed statins. Still, in Sweden, statins are recommended after ischemic stroke, regardless of stroke cause including AF.…”

“…Hyperglycemia increases stroke patients' risk of death by 87% at 30 days, 75% at 1 year, and 41% at 6 years after stroke, respectively (Johnston and Beckman, 2019). In addition to being an independent predictor of stroke-induced death (Geary et al, 2019), hyperglycemia is a determinant of stroke recurrence (Hotter et al, 2019). Hyperglycemia augments brain injury through multiple potential mechanisms, including endothelial dysfunction, oxidative stress, increment of inflammatory response, and impaired fibrinolysis (Garg et al, 2006;MacDougall and Muir, 2011;Nogueira-Machado et al, 2011;Krinock and Singhal, 2021).…”
