Secular Trends in Severe Idiosyncratic Drug-Induced Liver Injury in North America: An Update From the Acute Liver Failure Study Group Registry

Abstract: INTRODUCTION:Idiosyncratic drug-induced liver injury (DILI) is the second leading cause of acute liver failure (ALF) in the United States. Our study aims were to characterize secular trends in the implicated agents, clinical features, and outcomes of adults with DILI ALF over a 20-year period.METHODS:Among 2,332 patients with ALF enrolled in the ALF Study Group registry, 277 (11.9%) were adjudicated as idiosyncratic DILI ALF (INR ≥ 1.5 and hepatic encephalopathy) through expert opinion. The 155 cases in era 1 … Show more

“…In the present study, the pooled incidence curve for ATLI showed a clear upward trend worldwide, which is consistent with other research findings [ 44 , 45 ]. In recent years, with the extensive use of drugs and the accelerated research of novel drugs, the different types of clinical drugs have been continuously increasing, and the incidence of DILI has been rising year by year [ 46 ], and ATLI is no exception.…”

“…Previously, most of them had advanced coma grade ≥2 (68%) (117). Newer data suggest a reversal in this trend, with most (66.4%) presenting with lower-grade encephalopathy (118). Most patients have deep jaundice, with bilirubin levels generally >15 mg/dL.…”

“…12 Within the overall DILI group, the mix of drugs implicated has evolved; antibiotics remain the most common overall group, but the incidence of HDSs has doubled in the past decade from 10 to 20%. 13 Overall, ALF due to HDS are associated with worse overall outcomes than other forms of DILI and will be discussed in more detail below. Certain agents are less commonly used and thus less frequently implicated.…”
