Seizure Duration and Hemodynamic State During Electroconvulsive Therapy: Sodium Thiopental Versus Propofol

Abstract: Introduction:General anesthesia is required for Electroconvulsive Therapy (ECT) and it is usually provided by a hypnotic agent. The seizure duration is important for the treatment, and it is usually accompanied by severe hemodynamic changes. The aim of this study was to compare the effects of sodium thiopental versus Propofol on seizure duration and hemodynamic variables during ECT.Methods:A number of 100 patient-sessions of ECT were included in this randomized clinical trial. The initial hemodynamic state of … Show more

“…PRO has direct antiemetic effects 15 16 and several key benefits, including rapid onset of action, lack of active metabolites and rapid hepatic clearance after intravenous injection. 2 25 On the other hand, DEX is an adrenoceptor agonist, sedative and antihypertensive agent and its infusion reduces HR, systemic vascular resistance, and BP. It is an adjuvant to induce general anesthesia with a central sympatholytic effect, contributing to maintaining the patient’s hemodynamic status.…”

“…1 The procedure applies scheduled electrical stimulation to the central nervous system to initiate seizure activity. 2 Nowadays it has been employed for treating severe mental illnesses, chiefly, MDD, bipolar mood disorders, schizophrenia, and catatonia. 1 3 Electrical stimulations produce a generalized tonic seizure lasting nearly 10 seconds, followed by a generalized clonic seizure for a varied period ranging from a few seconds up to more than 1 minute.…”

“…20 Although methohexital and sodium thiopental were previously the most frequently used anaesthetic agents for ECT, these drugs have now been largely replaced by propofol due to its wider availability, faster recovery profile and favourable effect on haemodynamic stability, despite its effect in reducing seizure production. 19,21 Other inhalational anaesthetic agents have been compared favourably with propofol. 22 Ketamine and etomidate do not have anticonvulsant properties and seem to provide a higher quality and duration of seizures.…”

“…2,3 To enhance patient comfort and ensure safety, patients are typically given a short-acting general anesthetic agent, commonly propofol, as well as a short-acting paralytic. Propofol, a potent antiepileptic agent, 4 has been shown to decrease seizure duration in the setting of ECT, 5 which may reduce the efficacy of ECT as a depression treatment.…”
