Objective: The study was aimed to find out the incidence, obstetric risk factors, and various complications of abruptio placentae and to analyze the maternal and perinatal outcome of the same. Materials and Methods: It was a hospital based observational study for a period of one year carried out from 1 st June 2012 to 31 st May 2013. Study population included all pregnancy cases of gestational age of 28 weeks onwards having sign and symptoms of abruptio placentae. Results: Out of 15111 deliveries there were 70 cases of abruptio placentae. The incidence of abruptio placentae was found to be 0.46%. Associated risk factors were anaemia (51.4%), gestational hypertension (37.1%), previous caesarean section (28.6%), polyhydramnios, oligohydramnios etc. Vaginal delivery and caesarean section rates were 58.57% and 41.43% respectively. Eighty percent (80%) babies required NICU and 40% had early neonatal death. Conclusion: Maternal and fetal complications can be minimized provided patients report in time, so that prompt, judicious and definite measures can be undertaken expeditiously and correct treatment instituted.

Background Antepartum hemorrhage is a very serious problem and contributes significantly to maternal and neonatal morbidity and mortality in developing countries including Ethiopia. Identification of risk factors of antepartum hemorrhage will help for prevention and control programs. Methods An institutional-based case–control study was conducted from July 2013 to June 2016. A simple random sampling technique was employed, and 420 mothers’ medical cards were reviewed using a pretested checklist. The data were entered in to EPI-info version 7 and then exported to SPSS version 20 for analysis, and all variables with a P -value<0.2 at bivariate logistic regression analysis were considered as a candidate for multivariate logistic regression analysis, and those variables with a P -value<0.05 in multiple logistic regression analysis were considered as significantly associated at 95% confidence interval. Results Results of the multiple logistic regression showed that those mothers whose age was in the range of 35–44 years were significantly 4-times more likely to present with antepartum hemorrhage as compared to mothers in the age range of 15–24 years. Moreover, mothers who presented with a previous history of cesarean section were 4.7-times more likely to present with antepartum hemorrhage than those who had no previous history of cesarean section. Furthermore, women with a previous history of abortion were 2-times more likely to develop antepartum hemorrhage as compared to women who had no previous history of abortion. Conclusion and Recommendation In this study, the factors found to be risk factors of antepartum hemorrhage were maternal age, previous history of cesarean section, and previous history of abortion. Strategies towards reduction of unwanted pregnancy to prevent abortion, pregnancy at advanced age, and to reduce the cesarean section rate should be emphasized.

Aim. The aim of this study was to assess coagulation markers of endothelial damage and examine new markers of endothelial activation such as matrix metalloproteinases (MMPs) in a group of healthy pregnant women. Matrix metalloproteinase (MMP)-2, in particular, plays a major role in the degradation of the extracellular matrix confirming its essential function in both the survival (angiogenesis) and death of endothelial cells.Detection of specific coagulation factors, mainly released from the vascular endothelium such as vWF, sTM (soluble thrombomodulin) and ePCR (endothelial protein C receptor) and factors dependent on endothelial activation such as t-PA and PAI-1, could provide information on possible endothelial dysfunction and help differentiate pregnant patients with an altered thrombotic state.Methods. Healthy pregnant women underwent complete assessment for endothelial damage (as vWF, vWF activity, sTM, ePCR, EMP, MMP-2, MMP-9 and TIMP-2) using the ELISA and other methods.Results and Conclusions. The results show that endothelial activation during pregnancy is different from that in other pathological conditions involving endothelial damage and typically characterized by higher levels of both coagulation endothelial markers and MMPs. In pregnancy, changes in extracellular matrix composition and matrix metalloproteinase activity also occur and promote vascular remodeling but, only in the uterus. Predisposing risk factors for epithelial dysfunction, and vascular mediators associated with vascular remodeling must be assessed from concentrations in whole blood. The levels of MMPs are not increased in the circulation and the local situation in the uterus cannot be monitored this way.However, MMP-2 processes and modulates the functions of many other vasoactive and pro-inflammatory molecules including adrenomedullin, big endothelin-1, calcitonin gene-related peptide, CCL7/MCP-3, CXCL12/SDF-1, galectin-3, IGFBP-3, IL-1 Beta, S100A8, and S100A9. These molecules represent new potential molecular markers of endothelial damage during pregnancy.
