Selective and early increase of IL-1 inhibitors, IL-6 and cortisol after elective surgery

Abstract: SUMMARY After trauma, inflammatory, immunological and hormonal changes are well documented. Surgical intervention is a form of programmed trauma. Through the study of surgical patients, changes in early endogenous mediators of inflammation, immune response and tissue repair can be investigated. Here we analysed changes in serum levels of IL‐1 inhibitors, IL‐1β, IL‐6, tumour necrosis factor‐alpha (TNF‐α) and cortisol in patients undergoing elective surgery. C‐reactive protein (CRP) was measured as a marker of t… Show more

“…OM and OA individuals and returned to baseline levels by day 4; finally, TNF-SR75 inereased following surgery in OM and OA but not in RA, and returned to baseline by day 4. Di Padova et ai failed to demonstrate an increase in TNF-a following surgery, but found increased levels of lL-1 inhibitors during the first post-operative 24 h [3]. To the best of our knowledge, this is the first time that a laek of increase in detectable TNF-a has been shown and correlated with an early increase in TNF-sR following surgery.…”

“…Furthermore, it has been proposed that circulating levels of cytokines depend upon multiple factors including not just the type of stimulant but also the duration of exposure to the stimulant (Di Padova et al 1991). Hence, failure to detect significant changes in serum inflammatory markers may also be attributed to the short duration of the observed endotoxemia.…”

“…Tissue‐level responses to the inhibition of IL‐1β in wound healing have been investigated using IL‐1ra. Inhibitors of pro‐inflammatory cytokines are naturally produced following injury 38 but are insufficient to prevent rampant inflammation that is thought to be partly responsible for imperfect healing. An example of this is in corneal injuries, where healed tissue is often characterized by dense vascularization and loss of optical transparency.…”
