Self- and Informant-Reported Memory Complaints: Frequency and Severity in Cognitively Intact Individuals and those with Mild Cognitive Impairment and Neurodegenerative Dementias

Abstract: Given greater diagnostic specificity and internal consistency of informant report, clinicians should weigh this information more heavily than self-report in the diagnostic process.

“…Based on typical cognitive symptoms of AD and related dementia, we hypothesized that measures of fluid ability (i.e., declarative memory, executive functioning, attention, and processing speed) from the CBB and NIHTB-CB would most strongly differentiate between diagnosis groups as compared to measures of crystallized ability (e.g., vocabulary). Filipiak, Giordani, Heidebrink, Bhaumik, & Hampstead, 2018;Weintraub et al, 2018;Weintraub et al, 2009). Participants also completed the NIHTB-CB and CBB within 3 months of their NACC-UDS assessment, the results of which were not used as part of the consensus conference diagnostic process.…”

“…5,6 To augment the evaluation of functional status in those with cognitive impairment, a knowledgeable care partner is often consulted. There is support for the widespread use of a care partner report to accurately reflect cognitive and functional changes in Alzheimer's disease (AD), 7,8 although the quality of study partner reports vary, and care partners may overestimate functional performance specifically among those with higher cognitive scores. 9 Despite these reports in AD, to date little is known about the relationship between self and study partner reports concerning the performance of IADLs during the course of PD.…”

“…One study 34 only described overall demographic characteristics but not those concerning the patients who underwent awareness evaluation. Only one study 36 did not acknowledge the demographic characteristics of the participants. Patients attending outpatient clinics or hospitals were assessed in five of the studies, whereas only one included a community-based sample 36 .…”
