Self-Cannibalism (Autosarcophagy) in Psychosis

Abstract: Only nine previous cases of self- or auto-cannibalism (autosarcophagy) have previously been reported in the literature. Here, we report a 29-year-old man with psychosis and a history of polysubstance use who presented after his second attempt to self-cannibalize. This case raises questions about the underlying causes and dynamics of self-cannibalism in psychiatric illness and its relation to other types of self-harm behavior.

“…While cases of self-consumption are very rare and are associated with psychosis [ 39 ], it has been documented in the absence of severe psychiatric disease and substance abuse [ 40 – 42 ]. As the placenta is actually part of the infant, placentophagy (postpartum eating of the placenta) [ 43 ] is more correctly classified as cannibalism and not autosarcophagy.…”

“…Self-cannibalism is an extremely rare form of major self-mutilation [20,21]. Although there have been reports on self-cannibalism after self-mutilation in psychosis or its absence, none has reported a fatal case where this behavior was associated with multiple self-inflicted stab and cut wounds [17,20,[22][23][24].…”

“…Moreover, it could serve as an attempt to restore a sense of power or attention that is perceived as having been stolen, forfeited, or never achieved through extreme actions. Interestingly, the use of self-cannibalism to gain power has also been documented in the case of a 29-year-old homeless man [ 9 ]. This individual was admitted to a hospital after trying to consume his own finger, a gesture aimed at demonstrating his psychological dominance and distinctive nature and to discourage potential harm from others [ 9 ].…”
