Aims: Our main target was to investigate the relationship of blood pressure (BP) unawareness and poor antihypertensive drug adherence with the clinical outcomes of the stroke including hospitalization time, degree of disability, and mortality rate. Methods and results:In this cross-sectional study, we evaluated 530 eligible patients (male ¼ 313; female ¼ 217), aged 18 years and older who had a proven diagnosis of nontraumatic first-ever stroke and were referred to the Shahid Beheshti Hospital of Hamadan, Iran, during the period from March 2019 to September 2021. The prevalence of BP unawareness was 19.6%, and 31.8% of antihypertensive drug users (14.3% of all studied population) had poor drug adherence, in which, older age, male gender, marriage, rural residence, and smoking were associated with the lack of appropriate drug adherence. There was no significant difference between patients with diverse stroke types (ischemic or hemorrhagic) from the points of BP awareness and adherence to antihypertensive drugs; nevertheless, patients with a positive history of cardiac diseases had a significantly higher awareness of their BP status (P ¼ 0.037). BP unawareness was associated with poor clinical prognosis, and could significantly increase stroke mortality (P ¼ 0.001) and disability (P < 0.001) rates as well as the duration of hospitalization (P < 0.001). Moreover, those who survived the stroke (modified Rankin Scale < 6) had the highest odds to be aware of their BP status (adjusted odds ratio [AOR] ¼ 2.380 [95% confidence interval [CI] ¼ 1.39-4.07]). Additionally, nonsmokers (AOR ¼ 7.740), urban residents (AOR ¼ 3.314), and literate patients (AOR ¼ 2.092) had the highest odds of having appropriate drug adherence. Conclusion:Stroke mortality and morbidity rates can be significantly modified by persuading people to monitor their BP regularly and maximize antihypertensive medication adherence. In the meantime, increasing the literacy level in society and reducing the smoking rate can play important roles in achieving these goals.

Introduction Globally, hypertension is a significant public health concern and a leading cause of premature death. Since this disease is incurable, it is necessary to promote patients’ self-care behaviors to control it. The goal of this research was to identify the elements that influence self-care actions in individuals with hypertension, utilizing the Protection Motivation Theory (PMT) framework. Methods A study was carried out in Omidiyeh, located in southern Iran, using a cross-sectional design, involving 397 adults with hypertension (198 females and 199 males) who were selected from comprehensive health centers using a stratified random sampling technique. Data was gathered through a demographic data collection form and a questionnaire created by the researcher, which was based on the PMT. Data were assessed utilizing SPSS 25 and AMOS 24 software, and various statistical tests including one-way ANOVA, Pearson correlation, multiple regression, independent t-test, and structural equation modeling were conducted. Findings The variables of intrinsic reward ( r = 0.182), extrinsic reward ( r = 0.288), and response cost ( r = 0.126) showed a significant negative correlation with self-care behaviors. On the other hand, perceived susceptibility ( r = 0.212), perceived severity ( r = 0.110), self-efficacy ( r = 0.555), and response efficacy ( r = 0.424) demonstrated a significant positive correlation with self-care behaviors. Specifically, self-efficacy and response efficacy were the most powerful predictors of self-care behaviors, respectively. Conclusion The results showed that several factors can forecast self-care behaviors in patients with hypertension. Researchers are recommended to incorporate PMT in educational interventions and concentrate on self-efficacy and response efficacy constructs.

Background and Objective: Hypertension is a common disease, often asymptomatic and treatable. Left untreated, this condition often leads to fatal complications. The aim of this study was to investigate the status of self-care and its relationship with self-efficacy of patients with hypertension in Mashhad in 2019. Materials and Methods: In this descriptive cross-sectional study, 250 patients with hypertension in Mashhad in 1398 were selected by multi-stage method from among patients referred to comprehensive health care centers. Data collection tools were a three-part questionnaire including: demographic information and a standard questionnaire of self-efficacy and self-care of blood pressure. Data were analyzed using t-test, analysis of variance, Mann-Whitney, Kruskal-Wallis, linear correlation and regression by SPSS software version 20 with a significance level of 0.05. Results: Based on the findings, self-efficacy and self-care behaviors with scores of 40.43 ± 10.13 and 48.78±10.66 were relatively undesirable. There was a significant relationship between self-efficacy and self-care behaviors with job, body mass index, number of sessions of receiving blood pressure counseling, drug use, and history of illness in the father (p <0.05). It is worth mentioning that a positive and significant correlation was observed between the general scores of self-care and self-efficacy (P = 0.001) (rp = 0.82). Conclusion:Due to the significant correlation between self-care behaviors and self-efficacy and because these cases are very effective in preventing and controlling blood pressure. Therefore, it is recommended that educational programs for patients with hypertension to increase adherence to self-care behaviors and improve levels. Patient self-efficacy to be designed and implemented.
