The entrapment of foreign objects in the oral cavity is a glaring risk for its accidental aspiration. This paper details a case of a partially movable foreign body that was lodged in the primary mandibular second molar of a 5-year-old boy with autism. We opted for a conservative and risk-free management strategy as per the foreign body’s size and position. This case details a FORMAL (Foreign Object Removal with Minimal Aspiration Liability) technique whose application is justified when there is a high-risk of aspiration of the foreign body based on its location.The tooth was then managed by pulp therapy followed by stainless steel crown. Follow-up at 6-months and 12-months revealed the tooth to be asymptomatic.
