Self-rated health and mortality in the NHANES-I Epidemiologic Follow-up Study.

Abstract: The ability of self-rated health status to predict mortality was tested with data from the National Health and Nutrition Examination Survey (NHANES-I) Epidemiologic FollowUp Study (NHEFS), conducted from 1971-84. The sample consists of adult NHANES-I respondents ages years (N = 6,440) for whom data from a comprehensive physical examination at the initial interview and survival status at follow-up are available. Self-rated health consists of the response to the single item, "Would you say

“…The risk of non-differential misclassification is probably of minor importance in this study, because non-differential misclassification tends to attenuate true differences, and this study shows significant differences between the social participation/trust combination categories in the initial stages of the analyses. Self reported health status is a valid indicator of health and chronic diseases; it has been stated that self reported health is a valid indicator of health in middle aged populations (Miilunpalo et al 1997), strongly related to chronic diseases and physicians ratings of health status (Wilson and Kaplan 1995;Goldstein et al 1984), and cohort mortality in the USA (Idler at al. 1990;Grant et al 1995;Schoenfeld et al 1994), and Sweden The overlap between the sub-item "sports event" in the social participation item and the leisure time physical activity item is probably low, because the "sports event" subitem is designed to measure activities such as being a spectator of a soccer game, while the leisure time physical activity item measures the extent of physical activity (indirectly in terms of energy expenditure).…”

“…Given these limitations, the PILL has been shown to be associated with additional health indices, such as treatment seeking, activity restriction, and aspirin use (see Methods). Also, general physical symptoms and symptoms associated with particular organ systems are facets of health status that have demonstrated relationships to other indices of health, such as medical prognosis, health risk behaviors, and morbidity (Idler and Angel, 1990;Segovia et al, 1989). A similar limitation is that trauma-related sleep disturbance was measured by self-report, and objective documentation of sleep disturbance would be useful.…”

“…The following Health Indicator instruments were selected: Health Distress (measures worry and concern caused by chronic illness, 5 items [23]), Self-Rated Global Health (this item comes from the National Health Survey and has been found to be predictive of future health status, 1 item [23]), Activity Limitations (measures role function, 4 items [22]), Health Assessment Instrument (measures disability and is used in the National Health Survey, 20 [22,26]) and Hospitalizations (self report of number of times hospitalized in past 4 months [22,26]). …”
