Seminoma in undescended testis

Abstract: A testicular carcinoma in intraabdominal undescended testis can mimic other mesenteric or retroperitoneal mass. The identification of pampiniform plexus draining the mass can play an important role in diagnosis of testicular carcinoma in undescended testis. We report a case of seminoma in intraabdominal undescended testis, draining into pampiniform plexus and thrombus within it.

“…Furthermore, an abdominal testis is four times more likely to develop tumour compared with an inguinal testis. [5] Undescended testis may be located anywhere along the pathway of testicular descent but 66% are located distally to the external inguinal ring, 16% are in the inguinal canal and 10% remain in the abdominal region 3. Our patient’s undescended testicle was visualised within the inguinal canal although his tumour burden was intra-abdominal.…”

“…In an undescended testicular seminoma, clinical presentation may vary widely from asymptomatic mass to symptoms simulating appendicitis, mass effect symptoms or acute abdominal pain secondary to torsion or haemorrhage 3. One case reported a rapidly enlarging mass in the left lower abdomen, immediately above the left groin, initially thought to be a soft tissue sarcoma 5.…”

“…Undescended testes may be located anywhere along the developmental testicular descent pathway from the lower pole of the kidney to the external inguinal ring [5]. The etiology of undescended testis is multifactorial.…”

“…The diagnosis of seminoma is difficult in patients with undescended testis due to nonspecific complaints, such as abdominal pain and secondary symptoms from effects of the mass [5]. Several cases of germ cell tumors initially presented with acute abdomen have been reported previously, and in these cases acute abdomen occurred by torsion or bleeding of intraabdominal testes [25,26].…”

“…The intra-abdominal testicle is subject to complications such as cancer, ischemia and infertility 1 . The most common malignant transformation of undescended testicle is the seminoma 2 , 3 , 5 . Here is presented a case of adult massive pelvic mass corresponding to seminoma developed in intra-abdominal testis.…”
