Sensitivity and specificity of vessel wall MRI sequences to diagnose central nervous system angiitis

Abstract: Magnetic resonance imaging (MRI) with intracranial vessel wall (IVW) sequences is able to directly characterize disease processes affecting the VW increasing the accuracy of intracranial vasculopathies differential diagnosis. Nevertheless, data concerning the specificity and sensitivity of this technic for diagnosis of angiitis of the central nervous system (ACNS) are scant. We aimed at quantifying the IVW abnormalities in a cohort of primary and secondary ACNS and assessing the specificity of ACNS-associated … Show more

“…In three studies, 23,41,61 patients with a diagnosis of PACNS were selected for inclusion based on availability of HRVWI-MRI, thus the change in PACNS diagnostic accuracy due to HRVWI-MRI remains unknown. No study provided adequate information regarding the change in diagnostic accuracy provided by HRVWI-MRI when compared with DSA.…”

“…A study by Ferlini et al, 61 included patients with PACNS and secondary CNS vasculitis, and they presented limited data on PACNS subgroup. However, all patients with CNS vasculitis diagnosed by DSA had corresponding VWE.…”

“…Angiography is considered the gold standard in the imaging diagnosis of PCNSV. Currently, a diagnostic sensitivity of 90-100% is recognized for brain MRA, and its value in the diagnosis of angiitis is increasing [ 10 , 14 , 15 ].…”

“…The etiopathogenesis of RVCS in relation to ischemic stroke remains unclear [ 6 ]. Histopathological studies in patients with RCVS document minimal vascular changes, pointing to functional vasoconstriction rather than a vasculitis phenomenon characteristic of PCNSV, highlighting the increasing role of brain MRA as a noninvasive diagnostic option [ 14 , 15 ].…”

“…MRA can specifically examine blood vessels and help discover blockage and aneurysms (17). Timeof-flight MRA (Figure 1G) is the dominant non-contrast bright-blood method in grading residual aneurysm, and has been introduced for imaging human vascular system, and differential subsampling with an ultrafast high-spatialresolution head MRA (26,38,39).…”
