Sensitivity of the GEPARD Patient Questionnaire to Identify Psoriatic Arthritis in Patients with Psoriasis in Daily Practice: The GEPARD-Life Study

Abstract: Early detection of psoriatic arthritis (PsA) remains a challenge in clinical practice. Tools such as the German Psoriasis Arthritis Diagnostic (GEPARD) questionnaire have been developed for this purpose. The aim of this study was to determine the performance of the GEPARD questionnaire in the detection of PsA in psoriasis patients following rheumatology evaluation in daily clinical practice in Germany. This was a multicenter study involving 59 dermatology units (university/general hospital/office based), and t… Show more

“…There are many questionnaires available, including: Toronto Psoriatic Arthritis Screen II (ToPAS II), Psoriatic Arthritis Screening and Evaluation (PASE), Psoriasis Epidemiology Screening Tool (PEST), and Early Arthritis for Psoriatic Patients (EARP). 24,25 Sensitivities and specificities of questionnaires are well documented, 24,25 and studies comparing the effectiveness of questionnaire use to current standard of care at reducing delay in psoriatic arthritis diagnosis were identified as high priority.…”

“…The currently available screening tools for PsA have been validated in various clinical settings; these tools include the Early Arthritis for Psoriatic Patients (EARP), German PsO Arthritis Diagnostic questionnaire, PsO Assessment Questionnaire (PAQ), Psoriatic Arthritis Screening and Evaluation (PASE), PsO and Arthritis Screening Questionnaire (PASQ), Psoriasis Epidemiology Screening Tool (PEST), Toronto Psoriatic Arthritis Screening (ToPAS) and ToPAS 2 . For this analysis, the validated PEST questionnaire was selected because it is an effective tool that can be used in clinical practice, is easy to use (just five questions) and is available in the Corrona PsO Registry.…”

“…With the use of ultrasound, magnetic resonance, and scintigraphy, the rate of diagnosis of inflammatory joint manifestations increased to 79.6%. Considering the limitations in the availability of complementary methods, in 2016 the same authors presented a study in which they showed the usefulness of this questionnaire under conditions of usual clinical practice [16]. They recruited 1,512 patients with psoriasis in ambulatory dermatology centers (universities/general hospitals and doctor’s offices) throughout Germany, 52.9% (800/1,512 patients) of whom had ≥4 points in the GEPARD questionnaire.…”

The prevalence of PsA according to the CASPAR classification criteria was 33.8% (175/517 patients) of those patients with a positive GEPARD questionnaire [16]. Considering the importance of having simple tools that allow the detection of PsA in patients with psoriasis, we have developed a translation of the GEPARD questionnaire and evaluated its performance in Argentinian patients.…”
