Sentinel node biopsy as an adjunct to limb salvage surgery for epithelioid sarcoma of the hand

Abstract: The rarity of this tumor makes definitive conclusions difficult but SLN biopsy appears to be a useful adjunct in the treatment of these sarcomas.

“…As compared to other types of soft tissue sarcomas, the spread to regional lymph nodes can occur more frequently (> 20%) [ 75 , 76 , 77 , 78 ], but the most common site of metastasis is the lung and pleura [ 12 , 23 , 25 , 29 , 68 , 70 , 78 ]. This higher rate of nodal involvement may justify performing sentinel node biopsy in selected cases, but existing data show the low percentage of finding metastatic tumors in clinically noninvolved nodes [ 76 , 79 , 80 , 81 ]. In the case of the presence of lymph node metastasis, therapeutic lymph node dissection is indicated [ 23 , 67 , 78 ].…”

“…ES has a propensity for regional lymphatic spread and SLNB offers the advantage of a minimally invasive lymph-node staging procedure, which can improve the dismal prognosis and survival rates associated with this tumor. It also allows for the accurate early detection of micrometastasis, thereby sparing patients the morbidity associated with a regional lymphadenectomy [ 8 , 12 , 13 ].…”

“…Adjuvant chemotherapy (ifosfamide and doxorubicin) can be used in cases of advanced metastatic ES and can also be used as neoadjuvant therapy to reduce the tumor burden (shrink the tumor) before surgery. However, the role of chemotherapy in non-metastatic disease is unclear as the responses to chemotherapy are of short duration [ 12 , 15 ]. Future studies with long-term follow-up will be needed to establish the role of chemoradiotherapy in the treatment of ES.…”

“…Such reconstructions include the reconstructions of tendon, muscle, and nerve function and are increasingly being employed in trauma cases, yet are not standard of care in any STS. 43,44 Nerve reconstructions provide the opportunity to not only regain motor function, but also restore lost sensation. Moberg already proposed in the '50s that an insensate hand as good is as a non-functional hand.…”

“…1 Pediatric MPNST Malignant peripheral nerve sheath tumors in pediatric patients have been described previously. 32,[41][42][43][44][45] 5-year overall survival in children varies between 34.6% and 51%. 32,41,42,44,45 No institutional study has yet been able to find a difference in survival between pediatric and adult tumors.…”
