Objective. The clinical management of patients with T1-2 oral squamous cell carcinoma (OSCC) and clinically node-negative neck (cN0) continues to be controversial. We performed a systematic review of the literature to assess the effect of elective neck dissection (END) and neck observation (OBS) on the prognosis of patients with cT1-2 N0 OSCC. Study Design. PubMed, Embase, and Cochrane Library were searched for studies related to END and OBS in patients with cT1-2 N0 OSCC. The Mantel-Haenszel method was used to pool odds ratios (OR) for neck nodal recurrence and hazard ratios (HR) for survival. Results. END reduced the risk of neck nodal recurrence (OR 0.45; 95% confidence interval 0.32À0.63; P < .00001) in cT1-2 N0 OSCC. The disease-free survival (HR 0.52; 95% CI 0.42À0.63; P < .00001) was significantly higher in patients treated with END. However, END failed to significantly improve overall survival (HR 0.83; 95% CI 0.67À1.04; P = .10) and disease-specific survival (HR 0.87; 95% CI 0.48À1.57; P = .65) compared with management by OBS. Conclusions. A reduction in neck nodal recurrence and an increase in disease-free survival might support the need for END in early-stage OSCC with clinically N0 neck. Background The eighth international symposium for sentinel node biopsy (SNB) in head and neck cancer was held in 2018. This consensus conference aimed to deliver current multidisciplinary guidelines. This document focuses on the surgical aspects of SNB for oral cancer. Method Invited expert faculty selected topics requiring guidelines. Topics were reviewed and evidence evaluated where available. Data were presented at the consensus meeting, with live debate from panels comprising expert, nonexpert, and patient representatives followed by voting to assess the level of support for proposed recommendations. Evidence review, debate, and voting results were all considered in constructing these guidelines. Results/Conclusion A range of topics were considered, from patient selection to surgical technique and follow‐up schedule. Consensus was not achieved in all areas, highlighting potential issues that would benefit from prospective studies. Nevertheless these guidelines represent an up‐to‐date pragmatic recommendation based on current evidence and expert opinion.

Aim: The purpose of this study was to conduct a systematic review of the published literature to assess the state of the art of this procedure. Sentinel node biopsy (SNB) in oral squamous cell cancer (OSCC) is a novel and proven useful technique alternative to the neck dissection (ND) in the management of OSCC. Methods: The authors searched PubMed for literature in English published for the last five years, addressing this topic. Prospective studies articles were selected with at least thirty patients studied. Results: Of 235 studies found, 14 studies met the exclusion and inclusion criteria for this review. The studies selected focused on the role of the SNB in the OSCC, advantages compared to ND and its limitations, testing different solutions and innovations that could implement the conventional procedure. Meta-analysis studies and review articles were also selected in order to perform the introduction and support the discussion. Based upon these findings authors have tried to establish the state of the art of the SNB and authors have highlighted recent advances that improve the sentinel lymphatic node biopsy technique in the future. Conclusion: SNB is an excellent staging method in OSCC and an interesting alternative to ND. The authors show the most appropriate procedures recommended in the bibliography revised in a trend to depict the actual state of the art.
