Sentinel node evaluation in prostate cancer

Abstract: Sentinel lymph node (SLN) based pelvic lymph node dissection (PLND) in prostate cancer (PCa) is appealing over the time, cost and morbidity classically attributed to conventional PLND during radical prostatectomy. The initial report of feasibility of the SLN concept in prostate cancer was nearly 20 years ago. However, PLND based on the SLN concept, either SLN biopsy of a single node or targeted SLN dissection of multiple nodes, is still considered investigational in PCa. To better appreciate the challenges, an… Show more

“…For three-dimensional quantitative non-invasive in vivo visualization of the TDLNs [ 6 , 7 ] by functional 18 F-FDG-PET ( 18 F-FDG: 181.1 g mol −1 ), we used a dedicated small animal microPET scanner (Siemens Heathineers, Knoxville, TN).…”

“…Therefore, SLND, already a first choice procedure in melanoma, breast and penile cancers, is taken into consideration as an alternative [89]. According to the concept of metastatic spread of the tumor along the lymph drainage pathways, an assumption can be made that the absence of cancer invasion in the sentinel nodes is coequal with the lack of metastasis in other LNs [90]. The implementation of SLND would ideally prevent patients with PCa from overtreatment them with ePLND [91].…”

“…Sentinel lymph node biopsy (SLNB) theoretically detects the first LNs in the chain of lymphatic drainage of the primary tumor [ 221 ]. The expected benefits of SLNB are the reduction in the surgical time, cost, and potential complications, as well as an improvement of the staging by identifying unusual drainage roots [ 221 , 222 ]. SLNB was used in PCa in 1999 [ 223 ] and proved to be a valid method for N-staging with high diagnostic accuracy [ 224 , 225 , 226 ].…”
