Septic Thrombophlebitis with Persistent Methicillin-Resistant Staphylococcus aureus Bacteremia and de novo Resistance to Vancomycin and Daptomycin

Abstract: Persistent methicillin-resistant Staphylococcus aureus (MRSA) bacteremia is associated with significant risk of mortality, especially when it occurs while on appropriate antimicrobial therapy. We herein describe an unusual case of a patient with prosthetic aortic tissue valve, who suffered from central venous catheter related MRSA bacteremia with septic thrombus formation in the superior vena cava. MRSA bacteremia persisted despite removal of the catheter and appropriate antimicrobial therapy including vancomy… Show more

“…More than 99.9 % of infections are caused by bacteria that are classed as daptomycin susceptible by MIC testing [187]. Additionally, while there are reports of resistance developing during treatment [188,189], this is rare, and in many cases daptomycin treatment fails despite the infection being caused by a bacterium with an MIC classed as susceptible [190][191][192].…”

“…[4,5] It is important to recognize the high mortality rate associated with this condition, which has been reported to be as high as 16%. [1,6] The current treatment for LLS is based on empirical antibiotic therapy directed toward the most frequent pathogens. Management of IJV thrombosis is controversial, with some experts recommending anticoagulation as adjuvant treatment.…”

“…Strains with a high level of resistance to daptomycin are rare; however, the use of daptomycin as an alternative to vancomycin is limited due to the associated resistance of certain strains to both antibiotics [2]. Reports of associated resistance in vitro were followed by reports of daptomycin treatment failure in infections caused by VISA strains and vancomycin failure in cases of daptomycin resistance [3][4][5][6][7].…”
