Sequelae of injury to the heart caused by multiple needles

Abstract: A case is reported in which multiple needles were inserted into the heart by a patient, resulting in the unusual combination of a coronary artery-cameral fistula to the left ventricle, an intramural defect of the left ventricular free wall and a ventricular septal defect. This unique lesion was suspected on the basis of two-dimensional and Doppler echocardiography and contrast computed tomographic imaging. Its presence was confirmed during cardiac catheterization and cineangiography.

“…Also reported are coro nary arteriovenous fistulae, coronary artery aneurysm, myocardial infarction, and ven tricular septal defect [1,[6][7][8][9][10][13][14][15][16][17][18], Such foreign bodies are occasionally discovered on roentgenograms without any reported symptoms [ 14,15]. This is the first report of delayed cardiac tamponade and hemothorax due to a very' fine acupuncture needle which was barely detectable on the chest roentgen ogram.…”

“…[1] Hermoni et al [2] published a case report in which a 24-year-old man presented to an emergency department with acute-onset, sharp chest pain. Chest radiographs revealed 12 sewing needles, some of which were visible within the cardiac silhouette.…”

“…CT failed to confirm the exact number of needles, again probably due to cardiac motion during the CT exposure time (approx 0-09s). There has been one previous report of the use of CT scan for localization of intracardiac needles in which the CT was used to confirm the findings already obtained by echocardiography (Hermoni et al, 1986). There are several reports of the use of echocardiography for this purpose (Hsiung, 1985), (Hoshino et al, 1984), (Portek & Wright, 1981), and for the confirmation and localisation of other intracardiac foreign bodies (Fyfe et al, 1987).…”

“…Over 160 cases of injury to the heart by pins and needles have been reported (Hermoni et al, 1986). These objects reached the heart by a number of routes including embolization and penetration from adjacent organs after ingestion.…”
