Serial bone mineral density ratio measurement for fixator removal in tibia distraction osteogenesis and need of a supportive method using the pixel value ratio

Abstract: Distraction osteogenesis is one of the common procedures for limb lengthening. However, attempts are being made constantly to establish objective guidelines for early and safe removal of a fixator using a sensitive and quantitative measurement technique. Dual-energy X-ray absorptiometry (DEXA) has been evaluated in the past for understanding callus stiffness, and the present study is a step further in this direction. The purpose of this study was to evaluate the correlation between bone mineral density ratio (… Show more

“…Premature removal of the fixator after a lengthening procedure can result in gradual bending or acute fracture of the regenerate 15. Attempts are being made constantly to establish objective guidelines for early and safe removal of a fixator using a sensitive and quantitative measurement technique 16. The present study reports CT scan findings in patients undergoing limb lengthening.…”

“…Several techniques including CT scan, ultrasound, dual energy X-ray absorptiometry (DEXA), and Orthometry, have been used to evaluate the regenerate and determine when the external fixator should be removed. [14][15][16][17][18][19] Plain radiographs are, however, usually used to decide when the external fixation should be removed as this is simple and inexpensive. The incidence of early fracture (16%) in our series suggested difficulty in determining the timing of removal of the fixation by radiological appearances alone.…”

“…The fixator was removed in the clinic with mild oral sedation when the callus showed full corticalization in 3 cortices (when the pixel value ratio was equal to 1), [16][17][18] after which patients were given an abductor brace to use for approximately 3 months. Patients were asked to follow up every 2 weeks during the time when the fixator was applied, every 3 months for the first year, and then yearly thereafter.…”
