Serial Stent Implantation to Relieve In-Stent Stenosis in Obstructed Total Anomalous Pulmonary Venous Return

Abstract: A nine-year-old girl with complex cyanotic heart disease associated with supracardiac total anomalous venous return and obstruction at the drainage site of the vertical vein into the left innominate vein had stent placement at the age of seven, with immediate increase of oxygen saturation and improvement of physical activity. Significant in-stent stenosis occurred that was successfully treated by concentrical placement of a second stent. This case report demonstrates the transluminal approach to be effective i… Show more

“…166 Since then, the literature on pulmonary vein stenting has been limited to small series and tends to be divided into 2 major categories: Adults who acquire pulmonary vein stenosis after radiofrequency ablation therapy for atrial arrhythmias 174,194 -200,208 and children who either develop pulmonary vein stenosis after surgical repair of associated CHD 174 -176,178,184,187-189,213-218 or those with congenital pulmonary vein stenosis. 174,181,215,218,219 Other treatment modalities include surgery, 175,176,[183][184][185][186]188,216 balloon angioplasty, 177,179 and use of cutting balloons, 212,220,221 but to date, there is not a single procedure that produces a high rate of patency consistently and does not require reintervention.…”

“…179,180,184,187,212 Először 1991-ben írták le stentek alkalmazását a pulmonalis vé-nák szűkületének kezelésében. 166 Azóta a pulmonalis vénák stentkezelésével kapcsolatos szakirodalom csak kis esetszámú esetsorozat-vizsgálatokra korlátozódik és leginkább két fő kategóriát különít el: felnőttek, akikben a pulmonalis véna stenosisa a pitvari ritmuszavarok rádiófrekvenciás ablatióját követően alakult ki, 174,[194][195][196][197][198][199][200]208 és gyermekek, akikben a pulmonalis véna stenosisa a társuló veleszületett szívfejlődési rendellenesség sebé-szi korrekcióját követően jelentkezik [174][175][176]178,184,187,189,[213][214][215][216][217][218] vagy a pulmonalis vénák veleszületett szűkületében szenvednek. 174,181,215,218,219 Az egyéb kezelési lehetőségek közé tartozik a műtét, 175,176,[183][184][185][186]188,216 ballonos angioplastica, 177,179 és a vágóbal-lonok használata, 212,220,…”

“…[134] Stenting initially seemed very appealing, but restenosis has been universal. [132135136] As the transcutaneous approach may be difficult, especially in small children, intraoperative placement of stent may be an alternative mode of delivery. [52137] Unless stent placement is considered to be a strictly temporary measure, only stents that allow future expansion to adult dimensions (> 12 mm) should be used.…”

“…Since in-stent restenosis has been reported up to 2 years after implantation, repeat imaging (MRA or echocardiography) should be performed at regular intervals for at least the first 2 years. 15 After that, patients should be followed regularly, and any recurrent pleural effusions or other symptoms suggestive of obstruction should be investigated.…”
