Serological response to the 2009 H1N1 influenza vaccination in patients with inflammatory bowel disease

Abstract: Patients with IBD vaccinated with the 2009 H1N1 influenza vaccine had a low rate of seroprotection, particularly among those who were immunosuppressed. Although there is a need for studies of the clinical benefit of vaccines in this population, patients with IBD need to be aware of this reduced immunogenicity.

“…However, when patients are receiving immunosuppressive therapies with combined thiopurines and anti-TNF agents, serologic responses to vaccines are impaired ( 22,23,(43)(44)(45)(46)(47)(48)(49)(50)(51). Nonetheless, even a blunted vaccination response is thought to be of benefi t ( 52 ) and thus recommendations for infl uenza vaccination with inactivated vaccine continue to be broadly applicable to all patients, regardless of immunosuppression status ( 13,53 ).…”

“…Recent developments in immunomodulators or anti-TNF-a agents provide better prognoses for IBD patients, and several new immunomodulatory drugs, including biologics for individual target molecules, are under development in clinical trials [21]. Several guidelines recommend the annual influenza vaccination for IBD patients, especially those treated with immunosuppressive drugs, steroids, immunomodulators, or anti-TNF-a agents [26]. Educational intervention is effective for increasing the rate of vaccination [27], and the influenza vaccination is not associated with IBD flares [28].…”

“…Eine Impfung gegen HPV erreichte hingegen in einer kleineren Studie in einer IBDKohorte eine gute, mit Gesunden vergleichbare Immunogenität [123]. Bei der Influenza-Impfung findet sich für IBD-Patienten eine reduzierte Immunogenität [124]. Es sollte jedoch gegen Influenza geimpft werden.…”
