Seroprevalence of Antibodies That React with Anaplasma phagocytophila , the Agent of Human Granulocytic Ehrlichiosis, in Different Populations in Westchester County, New York

Abstract: We determined the frequencies of antibodies to Anaplasma phagocytophila, the agent of human granulocytic ehrlichiosis (HGE), in different groups of adults and children from Westchester County, New York. The groups included 159 adult blood donors and 215 children who were seronegative for Borrelia burgdorferi antibodies, 118 adult patients and 57 children who were seropositive for B. burgdorferi antibodies, and 42 adult patients with culture-confirmed erythema migrans. Eighteen (11.3%) of the blood donors and 1… Show more

“…The disease has been reported from many countries in temperate areas of Europe (Petrovec et al, 1997;Aguero-Rosenfeld et al, 2002;Skarphédinsson et al, 2005) and the United States (Dumler, 1997;Strle, 2004). However, the geographic distribution largely depends on the distribution and density of the tick vectors (Ogden et al, 2002).…”

“…Infection with A. phagocytophilum is widespread in the USA (Dumler, 1997;Strle, 2004) and in Europe (Petrovec et al, 1997;Aguero-Rosenfeld et al, 2002;Skarphédinsson et al, 2005), and has been reported in countries neighbouring Turkey, such as Greece (Daniel et al, 2002), Bulgaria (Christova and Dumler, 1999) and Iran (Bashiribod et al, 2004).…”

“…Acute and convalescent serologic testing using an indirect fluorescent antibody method for A. phagocytophilum IgG with demonstration of four-fold change or seroconversion is the most sensitive confirmatory laboratory test, and has been used most commonly to confirm HGA. 25,38,[54][55][56] Specific IgM tests are only reactive during the first 40 days after infection, and are less sensitive than those that detect IgG antibodies, even during this early interval. 57 Once a patient becomes seroreactive, antibodies can persist for months or years in the absence of any clinical or laboratory-based evidence for ongoing infection; thus, reductions in antibody titers cannot be used as monitors of effective treatment.…”

“…The most widely used and reliable test is the two-step serological diagnosis with the confirmation by Western blot in case of positive or doubtful serology. Serological findings, especially in endemic areas, are not always signs of the current infection and a negative serology is not sufficient to exclude the disease [13,14]. The positive serological result only proofs that infection has taken place, but cannot prove the disease and there may be seronegativity in case of co-infection with long lasting chronic disease.…”
