Background: Toxoplasmosis is a zoonotic disease that is asymptomatic in many but can spread through the placenta during pregnancy causing miscarriage, stillbirth, intrauterine death, and neurologic impairments. This study reports on the proportion and risk factors associated with the seropositivity of Toxoplasma gondii among pregnant women visiting the antenatal care unit of the Nkawie-Toase Government Hospital, Ghana. Methods: This was a cross-sectional study among pregnant women attending antenatal care at a district hospital in Ghana. The participants were selected randomly for the study. A structured questionnaire was used to collect information and data on possible risk factors associated with toxoplasmosis. Serum samples from the participants were screened for exposure to T. gondii using a commercial ELISA kit. The associations of the risk factors with seropositivity were tested using logistic regression. Results: There were 84 women who participated in this study. The mean age of the sample was 26.96 ± 5.67 years; 64.29% had basic education, 21.43% were traders and 14.29% were farmers. Forty-seven women (55.95%) tested seropositive for T. gondii, 44 (52.38%) for IgG and 3 (3.57%) for IgM. Women involved as traders (OR 18,, p = 0.003) and farmers (OR 6.75,, p = 0.033) had significantly higher risk of testing seropositive. Conclusion: High proportions of pregnant women in this study were found to be exposed to T. gondii. To prevent negative pregnancy outcomes, pregnant women attending antenatal care be educated and routinely screened for T. gondii infection.

Seroprevalence of Toxoplasma gondii infection among immigrant and native pregnant women in Eastern Spain

In European countries, toxoplasma antenatal screening is recommended to prevent toxoplasmosis. The seroprevalence of these infections in immigrants can be different than in native population. From February 2006 to June 2010, a cross-sectional study was carried out in all pregnant women attended at a reference unit in Elche, Spain. An enzyme immunoassay was used for detection of IgG antibodies against Toxoplasma gondii. For each immigrant woman, one Spanish pregnant woman of the same age cared for in the same day was recruited (Spanish control group). A total of 1,627 migrant pregnant women participated in this study. The adherence to screening among migrants was 91.9% (95% CI, 90.5-93.1%), similar than that found in Spaniards (92.2%; 95% CI, 90.8-93-4%). Among migrant women, 619 were positive for IgG anti-T. gondii antibodies (41.4%; 95% CI, 38.9-43.9%), compared with 12.0% (95% CI, 10.5-13.8%) among Spaniards (odds ratio (OR), 5.2 (95% CI, 4.3-6.3). Seroprevalence in pregnant women from Latin America, northern Africa, Eastern Europe, Africa Sub-Saharan and Western Europe was higher than in the Spanish control group (OR, 5.4, 5.8, 6.5, 5.4, and 2.4, respectively; p < 0.001). No Asian pregnant woman was immune. Seroprevalence increased with increasing age in migrant pregnant women: 15-25 years, 38.2%; 26-35 years, 40.7%; and 36-45 years, 52.8%. The seroprevalence of T. gondii infection in migrant pregnant women living in Spain was higher than in the native population. However, no cases were found in Asian immigrants, highlighting the importance of primary prevention of this infection in pregnant women coming from that geographic region.

The aim of the study was to evaluate the performance of the ImmunoComb® Toxo IgG and ImmunoComb® Toxo IgMassays (rapid diagnostic test) in the laboratory diagnosis of toxoplasmosis in pregnant women in Cotonou. We interviewed 266 pregnant women, who first answered an epidemiological questionnaire, and collected blood samples for measurement of IgG and IgM anti T. gondii antibodies with the ImmunoComb toxo assays and with the ARCHITECT CIMA method. The sensitivity, specificity, positive predictive values (PPV) and negative predictive values (NPV) were calculated to determine the performance of the rapid test. The seroprevalences of IgG against T. gondii by CIMA technique and rapid test were respectively 48.9% and 48.5%. The prevalence increased with age. Performances for IgG were: sensitivity 97%, specificity 100%, PPV 100%, NPV = 97.10%. For IgM, Sensitivity: 33.3% Specificity: 100%, PPV 100%, NPV = 99.2%. Seroprevalence obtained shows that about half of the study population is not immune against T. gondii and requires regular serological monitoring until delivery. According to these results, and given the needs of toxoplasmosis diagnosis on the field characterized by an important decrease of immunized women, this test may be recommended in the laboratory diagnosis of toxoplasmosis in peripheral levels of the health pyramid.
