Sertraline-Induced Anorgasmia Treated With Intermittent Nefazodone

Abstract: Sir: Hallucinogen persisting perception disorder is the reexperiencing of the perceptual symptoms experienced while intoxicated with the hallucinogen. Examples of these perceptual distortions include geometric hallucinations, false perceptions of movement in the peripheral visual fields, flashes of color, positive afterimages, macropsia, and micropsia. Various pharmacologic treatments of "flashbacks," including neuroleptics and benzodiazepines, and electroconvulsive therapy have met with limited success. 1 Cas… Show more

“…In theory, orgasm might be improved with medications that block 5HT-2 and 5HT-3 receptors, or enhance dopaminergic transmission. Indeed, the following agents have been studied as antidotes: the alpha 2 -adrenergic receptor antagonist, yohimbine; 33 the 5-HT 2 receptor antagonists, nefazodone 34 and cyproheptadine; 35 the 5-HT 3 receptor antagonist, granisetron; 36 the dopamine agonist, amantadine; 37 bupropion; 38,39 buspirone, 40 psychostimulants 41 and other agents, such as Ginkgo biloba and l-arginine (which increase NO) 42 and the phosphodiesterase type 5 inhibitors sildenafil, tadalafil and vardenafil. 30,43 Some agents with established antidepressant properties, such as bupropion, nefazodone and buspirone, may also be effective antidepressant augmentations, and improve sexual function indirectly via their effect on mood.…”

“…Unfortunately, most antidepressants create or increase sexual dysfunction. The selective serotonin reuptake inhibitors (SSRIs) [Waldinger et al, 1998;Seagraves, 1989;Patterson,or no sexual dysfunction [Crenshaw and Goldberg, 1996;Feiger et al, 1996;Reynolds, 1997].…”

“…Attempts to treat antidepressant-induced anorgasmia with cyproheptadine, amantadine, methylphenidate, yohimbine, dosage reduction, drug holidays and drug substitution had mixed results. 1 We report a case of amitriptyline-induced anorgasmia reversed by switching over to nefazodone.…”

Prior to the onset of depression he was sexually active.…”
