Serum 25‐hydroxyvitamin D increases with NB‐UVB and UVA/UVB phototherapy in patients with psoriasis and atopic dermatitis in Western Australia

Abstract: NB-UVB and UVA/UVB phototherapy significantly increased 25(OH)D serum level in patients with psoriasis and atopic dermatitis in Western Australia. Our study cohort had a higher baseline vitamin D level and a lower percentage increase of serum 25(OH)D post-phototherapy than the increases reported in the literature from cohorts in the northern hemisphere.

“…NB-UVB (narrow-band ultraviolet B light) and UVA/UVB phototherapy, widely used in the treatment of psoriasis, are thought to mediate its beneficial effect in part by elevating the serum 25(OH)D level [72,73,74]. A clinical trial compared the efficacy and safety of various treatment regimens for psoriasis (calcipotriol monotherapy, NB-UVB phototherapy alone and combination of calcipotriol and NB-UVB) and demonstrated that the combination of calcipotriol and NB-UVB twice a week was superior to other treatment regimens in rapidly reducing the PASI score of patients [75].…”

“…In another study, Drozdenko et al [139] showed that the oral intake of vitamin D increases the frequencies of CD38+ B cells to enhance the B-cell receptor-mediated response and decreases the IFN-γ and IL-17 T-cell cytokine response in vitamin D-deficient individuals. Additionally UVA and UVB phototherapy is widely used in AD treatment because of its effects in the T cell-mediated immune response, and it is suggested that the beneficial effect of UVA/UVB phototherapy is also mediated by the correction of the vitamin D deficiency or insufficiency in AD patients [72]. An increased IgE response to common environmental and food allergens is a common feature in AD.…”

“…Skin biopsies from baseline and at week 4 of nbUVB phototherapy were analysed with RT qPCR for selected cytokine mRNA. This showed no change in IL-10, IL-13, IL-22, IL-33, or IFN-γ mRNA following nbUVB phototherapy ( (11,(21)(22)(23)(24)(25)(26). The lack of effect of FLG loss-of-function mutations on this increase contrasts with epidemiological and experimental studies that have indicated that decreased filaggrin levels in the skin cause higher nbUVB penetration (14)(15)(16)(17)(18)(19).…”

“…The beneficial effect of phototherapy on atopic dermatitis has been associated with positive contributions to vitamin D deficiency or insufficiency. 18,19 In a meta-analysis study, vitiligo was associated with vitamin D deficiency and topical vitamin D analogues and phototherapy have been shown to be effective in the treatment. 20,21 Alopecia areata is a T-cell mediated disease targeting the hair follicle.…”
