Serum adipocytokines are related to lipodystrophy and metabolic disorders in HIV-infected men under antiretroviral therapy

Abstract: These results suggest that adiponectin and the TNFalpha system are related to lipodystrophy, insulin resistance and metabolic alterations in patients under PI-based HAART. A/L and sTNFR1 could predict insulin sensitivity and potential cardiovascular risk in these patients.

“…In the present study, we further examined the effects of leptin on apoM synthesis in HepG2 Leptin is a cytokine-like peptide secreted by adipose tissue, known to regulate hepatic lipid and lipoprotein metabolism [12][13][14][15]. Mice with genetic defects in leptin exhibit hyperlipidemia, hyperinsulinemia, hyperglycemia and resistance to insulin [16][17][18].…”

“…Adipocytes secrete a range of adipocytokines which control insulin sensitivity 66 . There is evidence that an adipocytokine, adiponectin, a protein product of the apM1 gene, which is expressed exclusively in adipocytes, plays a role in development of HIV-associated lipodystrophy as well as in congenital and acquired lipodystrophies in non-HIV infected subjects 67 .…”

“…In vitro and animal studies and cross-sectional studies in humans have shown that adiponectin is inversely correlated with features of HAART-associated metabolic syndrome. This syndrome has recently been linked to a quantitative trait locus on chromosome 3q27, the location of the apM1 gene 66,68,69 . These studies have shown that both adiponectin levels and the adiponectin-to-leptin ratio are positively correlated with features of HAART-associated metabolic syndrome 66,68,69 .…”

“…A combination of adiponectin and leptin as an index of insulin sensitivity was first proposed in 2003 [21]. Subsequently, many investigators have reported that the adiponectin/leptin ratio (or leptin/adiponectin ratio) is a relevant and reliable marker of insulin resistance in comparison with other parameters [20].…”
